On 05/17/2024 at 9:10 AM an unannounced Annual/Random Inspection was conducted by Licensing Program Analyst (LPA) Katrina Chicote. LPA announced purpose of visit and met with Facility Representative, Luciana Arevalo, Office Manager. Facility is currently licensed for a capacity of 184, per FR they currently have 59 enrolled. The program currently operates Monday thru Friday from 7:00 AM to 5:30 PM. LPA singularly toured facility both indoors and outdoors. LIC 125 was provided.

The facility is made up of four classrooms (Rm-1, Rm-2, Rm-3, and Rm-4), but per FR they are currently only using three classrooms (Rm-1, Rm-2, and Rm-4) due to low enrollment. At the initial start of the site visit, LPA observed a total census of 17 children with five staff in Rm-1, 21 children with two staff in Rm-2, and 12 students with five staff in Rm-4. Total census for the day was 50 students with 12 staff at time of inspection.

Beginning at 9:15 AM, furniture and equipment were inspected for age appropriateness and good repair in each classroom. LPA observed surface of classrooms to be safe and clean, and child size tables and chairs available for children well maintained. LPA observed learning stations for dramatic play, math, and science with toys well maintained free of loose and sharp parts and a library area with soft seating available for quiet play. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings. Cots were inspected and observed stored in closets of each classroom. Per staff, linens are provided by the facility and washed weekly by a laundry service. Availability of drinking water was reviewed. LPA observed drinking fountains available for children in each classroom, per Staff, drinking fountains are used to refill children's personal water bottles. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. LPA observed three bathrooms available for facility totaling six toilets, two urinals, and six sinks. LPA observed additional sink available in each classroom.

LPA observed a first aid kit is kept in each classroom which were inventoried for necessary supplies. LPA tested carbon monoxide detector which was observed to be in operable condition at time of inspection. LPA observed smoke detectors and fire extinguishers present at the facility. Disaster drill log was available and last drill conducted was on 05/2024. Sick/isolation area is in Office Lobby. Hazardous items including poisonous cleaning compounds are not present inside classrooms per LAUSD requirements. Janitorial services cleans facility daily. Per a variety of staff interviewed, no children at the facility currently require medications.

Snack/lunch menus were reviewed and observed posted. Food preparation areas were toured for safety, cleanliness and proper equipment. Facility provides Breakfast, Lunch, and Snack which is delivered by nearby elementary school campus.

Beginning at 9:30 AM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. LPA observed four outdoor play areas available for children. Per staff, Outdoor Play Area 1, which is accessible through door in Rm-4, is currently not being used. LPA observed other outdoor play areas to each have an age appropriate play structure with cushioning material below to absorb fall that have both been well maintained at time of inspection. LPA observed a variety of learning stations for dramatic play and manipulatives with age appropriate toys available for children free of loose and sharp parts. LPA observed benches with seating available for children through out and a library area with appropriate seating available for children. LPA observed bicycles available on concrete bike path with a surface that has been well maintained for activity at time of inspection. Required shade and fencing were inspected. LPA advised that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to drinking fountains and their water bottles during outside play. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed, LPA observed all children present signed in at time of inspection. Personal Rights of children were observed by LPA.

All staff and employees have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Los Angeles Unified School District (LAUSD).

Beginning at 10:20 AM, children and staff records were reviewed. Children’s records were reviewed for completeness and documented on the LIC 857. 6 of 6 children's records were reviewed complete with required documents. Staff records were documented on the LIC 859. 6 of 6 staff files reviewed did have proof of current AB1207 Mandated Reporter Training certificate on file at time of inspection. There is at least one staff member trained in CPR and Pediatric First Aid present during this inspection. CPR card expires on 02/2025 for Staff #2. LPA did not review confidential forms and information due to the files being stored at the administrative office.

The following were discussed:
LPA advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation, this includes any changes as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location.

LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES:
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

MyChildCarePlan.org – Centers and Family Child Care Homes Facility Representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today, 05/17/2024.

A notice of site visit was given and must remain posted for 30 days.
