On 01/07/2026 at 9:10 AM an unannounced Annual/Random Inspection was conducted by Licensing Program Analyst (LPA) Claudia Kam. LPA announced purpose of visit and met with Lilliana Carrasco Facility Representative (FR). Facility is currently licensed for a capacity of 91 and per Facility Roster currently have 61 enrolled. The program currently operates Monday thru Friday from 7:00 AM to 5:30 PM. LPA did a self guided tour of the facility both indoors and outdoors. Required postings were observed. LIC 125 Entrance Checklist was provided.

The facility is made up of four classrooms: RM-1, RM-2, RM-3 and RM-4. At the initial start of the site visit, At initial start of the visit LPA observed was 38 children with 12 staff at time of inspection. Currently due to floor repair children from room 1 have been combined with room 3 (ages 2-3) and children from room 2 are combined with room 4 (ages 3-4).

Beginning at 9:10 AM, furniture and equipment were inspected for age appropriateness and good repair in each classroom. LPA observed surface of classrooms to be safe and clean, and child size tables and chairs available for children with rounded edges and well maintained. LPA observed learning stations for dramatic play, math, and science with a variety of age appropriate toys maintained free of loose and sharp parts and a library area with soft seating available for quiet play. Telephone service, heating, lighting and ventilation were evaluated. Cots were inspected and observations discussed. Per FR linens are provided by the facility and washed by weekly laundry service. Cabinets were observed unlocked or non functioning, no hazardous items were observed, observations discussed.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Availability of drinking water was reviewed. LPA observed drinking fountains available for children in the classrooms as well as children's personal water bottles available. There were cubbies observed as storage for children's belongings in hallways by the bathrooms.

Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. LPA observed 4 bathrooms available totaling 9 toilets, 3 urinals, 2 changing tables and 2 trough sinks and 6 individual sinks. Hazardous items including poisonous cleaning compounds were stored inaccessible to children in the off limits kitchen area. Per FR, only LAUSD approved disinfectants are present at the facility, LPA observed these items stored in the kitchen back room and observed locked at time of inspection.

LPA observed first aid kit is kept on mobile carts that can be taken outside and one in the classroom cabinet in each room. LPA observed First Aid Kit supples in closet. Per FR, they currently have one child (C5) that require medication. LPA observed medication stored in front office filing cabinet which is centrally located but inaccessible to children as it requires a key to open. LPA observed medication for children in original packaging with unaltered label and conducted record review to ensure completion of required documents.

At 9:40 AM, LPA observed carbon monoxide detector present in the Kitchen. Detector was tested and found to be operable. LPA observed smoke detectors and fire extinguishers are present in the facility at time of inspection. Disaster drill log was observed posted and last drill conducted was on 12/05/2025. Isolation area for sick children is in the cozy corner of the classroom where child will have full supervision until child is picked up by Authorized Representative.

Snack/lunch menus were observed posted. Facility provides Breakfast, Lunch, and PM Snack. Food is prepared and delivered by Dacotah Elementary and distributed at the facility. Food staging area was toured for safety, cleanliness and proper equipment. LPA observed trash cans with tight fitting lids available at time of inspection.

Beginning at 9:50 AM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. LPA observed two outdoor play areas available at time of inspection. Currently due to construction, playground for room 1 and room 3 are not in use as they are being used to store classroom furniture. Furniture and equipment was observed covered and cordoned off. LPA observed surface of outdoor play areas to be safe and maintained for activities planned such as concrete bicycle path and cushioning material below multi-level play structures. LPA observed multi-level play structure to have climbing equipment and slide that is age appropriate and maintained.

LPA observed a variety of age appropriate toys available for dramatic play, manipulative toys, and art that have all been well maintained and made for outdoor use. Required shade and fencing were inspected. LPA observed a variety of shaded areas available for children with seating available at time of inspection. LPA advised that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to drinking fountains and their personal water bottles during outside play, which LPA observed available through out the play yard. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed and discussed. Personal Rights of children were observed by LPA.

Beginning at 10:30 AM, staff and children’s records were reviewed for completeness. Inspection of required forms was made and documented on the LIC 857 for children and the LIC 859 for staff. LPA did not review confidential staff files during the inspection due to the staff confidential files being stored at the administrative office. CPR card expires on 10/2027 for Staff #1. 9 of 10 staff files reviewed did have proof of current AB1207 Mandated Reporter Training certificate on file. All staff and employees have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Los Angeles Unified School District (LAUSD).

LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

The following were discussed with the Facility Representative:

The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked, and batteries replaced as needed. Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. The Director was also advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCLD within the time frame specified by the regulation.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.
To receive important licensed related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-care-licensing/subscribe and select the Child Care option to receive email communication.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP.).

LPA verified that the lead testing was completed in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP.


This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today 1/7/2026.
