Licensing Program Analyst (LPA) Saul Valenzuela conducted an unannounced Case Management inspection due to two incidents that were reported to the Department on 10/18/2024 and 2/25/2025. LPA met with Principal Meliza Rodriguez who guided LPA on a tour of the facility. Census was taken.

On October 18th, 2024, an incident was self-reported to the Department via Email by the facility who reported a staff disclosed that a child’s personal right was violated by another staff.



On February 25th, 2025, an incident was self-reported to the Department via Email by the facility who reported that a child sustained an injury that requires medical treatment.

All reports were reported within the required 24 hours. The purpose of the inspection was to obtain additional information regarding the incidents reported to the Department.

Regarding incident that occur on October 18th, 2024 – Staff #1 disclosed that Child #1 was pulled out of the chair by Staff #2 to be placed on the carpet. According to Staff #1, Child #1 did not want to sit on the carpet and was holding onto the chair. Staff #1 stated that Staff #2 pulled on he chair until Child#1 released the chair. Staff #2 lifted them by the arms and placed child on the carpet. According to Staff #1, Staff #2 held Child #1 on the carpet until Child #1 stopped trying to get away. LPA was unable to interview Child #1 at this time.

Regarding incident that occur on February 25th, 2025 – staff statement corroborated that incident was observed. Child #2 was running during outdoor play time when child tripped and fell hitting their elbow on the ground. Staff provided first aid immediately and notified parents. According to the Principal, child has not been taken to the doctor by the parents and child has not been back to school.

LPA obtained copies of staff statements.

Based on the LPA’s interviews and records review, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.



A notice of site visit was given and must remain posted for 30 days.

Exit interview conducted and report was reviewed with facility representative, Meliza Rodriguez.

Per Principal, they will have a staff meeting to review Personal Rights and conduct training on approaches to children who are not cooperating. Signed attendance sheet and copy of meeting agenda will be sent to LPA by POC due date.

Based on interviews adn staffs' statements, Licensee did not sure that Child #1 was accorded dignity, as staff disclosed that Staff #2 pulled on Child #1 against their own volition. This is a potential risk to the health, safety, and personal rights of children in care.
