On 10/24/24 at 12:35 pm Licensing Program Analyst (LPA) Claudia Kam conducted an unannounced annual required inspection at the above facility. LPA met with Dr. Elizabeth Blackwell, Facility Representative (FR) who guided analyst on a tour of the facility.

There were 46 children and 15 staff present when LPA arrived. Facility capacity is in compliance with license and Title Five ratio guidelines. All individuals present have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Los Angeles Unified School District.

LPA toured Classrooms 1 (21 children and 5 staff), 2 (toddlers) (14 children and 4 staff) and 3 (11 children and 6 staff). All classrooms have a science corner, quiet time area, reading nook, arts and crafts area, and free play/circle time area. LPA observed cubbies with wooden toys, puzzles, books, and other appropriate toys to be clean and free of hazards at time of inspection. LPA observed carpets and age-appropriate tables and chairs in each classroom. Water is made readily available via water fountain in Room #1. Children in all classrooms bring water bottles from home and may use the fountain outside when playing or in room 1. Fountain in room 1 is equipped with a spout to refill water bottles. Per FR, there are no children with medication enrolled and no IMS is provided. Children have cubbies to store personal belongings separate from each other. Children were observed to be napping at time of visit. All children had an individual nap cot or mat and were being observed by staff. Bedding is provided by facility and washed weekly by laundry service California Linen Service. Extra bedding is kept at the facility for changes as needed. LPA observed red backpacks for first aid kept in each classroom.

LPA toured the children’s restrooms located behind Room #1. Restrooms were observed to be safe and sanitary with operable sinks and toilets.

Sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, safety, and sanitation. There are 3 bathrooms a boys bathroom with 2 urinals and 1 toilet, 2 sinks, and 1 bathroom with 3 toilets and 2 sinks and a third bathroom for with 2 toddler toilets and a trough sink.

LPA toured the outdoor play areas. Outdoor areas are split in two and designated for specific classrooms. The playground to the left is for classrooms 1 and 3 and the playground on the right side of the building is for classroom 2. Outdoor areas were observed to have age appropriate toys and material for children: reading nooks, basketball, play structure, and lounge areas, all areas were observed to be free of loose, sharp, and/or pointed parts. LPA observed required cushioning under climbing structure to absorb fall. Shade was observed throughout the outdoor area and water was observed to be readily available via water fountains. Children also take their water bottles outdoors as needed. LPA did not observe and hazards in the outdoor area.

LPA observed required posted documentation which included, Facility License, Publication (PUB) 393- Notification of Parent Rights, Licensing Form (LIC) 610- Facility Disaster Plan, PUB 269- Child Passenger Restraint System, LIC 613A- Notification of Personal Rights and Lunch/Snack Menu. All documents were observed in the main entrance.

Facility records were reviewed for LIC 9040- Facility Roster, 9148- Earthquake Preparedness form, Daily schedule and Disaster drill log, last drill conducted on 10/17/24. All documents were observed.

LPA reviewed Sign In/Out sheets located in each classroom. All children present were signed in with the full signature of guardian. FR was advised to remind parents to include the time when signing the child in/out.

Children’s records were reviewed for Emergency Card, Immunization Records, Licensing Form (LIC) 627- Consent for Medical Treatment, LIC 995 Notification of Parents’ Rights, LIC 701- Physician’s Report, LIC 613A- Personal Rights, and signed Admissions Agreement. LPA observed required signed documentation for medication. All files reviewed were complete.

Staff records were reviewed for approved Pediatric First Aid and CPR certification, LIC 9052- Employee Rights, Proof of immunization against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse, Transcripts or Permit and current Mandated Reporter Training Certificate. All staff files reviewed were complete.

The following were discussed with the Director:
The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked, and batteries replaced as needed. Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. The Director was also advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCLD within the time frame specified by the regulation.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Center was reviewed for lead compliance and center meets standards for lead. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today 10/24/24.

A notice of site visit was given and must remain posted for 30 days.
