An unannounced Required – 3 Year Inspection was conducted on 11/17/2023 at 12:50 PM by Licensing Program Analysts (LPAs) Franchesca White and Katrina Chicote. Facility is currently licensed for a capacity of 82. Licensing staff met with Rosalia Nava . The program currently operates Monday thru Friday from 7:30 AM to 5:30 PM. LPA was taken on a guided tour of the facility of both indoors and outdoors. Disaster drill log was readily available during today's inspection, and LPA observed last fire drill to be conducted on 11/8/2023. All required postings were observed posted on a bulletin board upon entry.
At the initial start of the site visit, there is a total census of 58 children and a total census of 10 staff members present. The facility is made up of 4 rooms (Room 1, Room 2, Room 3, Room 4) all with a mixed age group of 3-5 year olds. Room 1 had 20 students and 3 staff, Room 2 had 11 students and 2 staff, Room 3 had 13 students and 3 staff and Room 4 had 13 students and 2 staff. LPAs observed each classroom for furniture and equipment were inspected for age appropriateness and good repair. LPAs observed shelves in all classrooms filled with plastic manipulative toys, wooden and soft blocks, quiet reading area, dramatic play, art area, and other age appropriate toys free of lose and sharp parts. LPAs observed child appropriate tables and chairs and linoleum floors with carpeted areas to be clear and free of hazards in each classroom at time of inspection. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings in each classroom. Mats/cots were inspected, facility uses standard cots. LPAs observed these items being stored in storage area each with children's names labeled. Director states that bed sheets and blankets are provided by the families, kept in children's backpacks, and taken home weekly. Availability of drinking water was reviewed, LPAs observed both plastic water bottles and reusable water bottles provided by the children. Facility water fountains are taped and blocked off to prevent use since COVID-19 health and safety guidelines were placed in effect. Age-appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. Room 1 has it's own Bathroom connected to classroom with 2 toilets and 2 sinks, Room 2 uses bathroom across from room in hallway that is designated for children.

with 1 toilet and 2 sinks, Room 3 has two bathrooms inside classroom totaling 4 toilets, 4 sinks, Room 4 has access to its own bathroom that has 2 sinks and 2 toilets. Room 4 Bathroom is jack and jill bathroom that has access to Room 3 made inaccessible with a four foot storage shelving case. LPAs observed each bathroom to have operable toilets and sinks, adequate handwashing supplies with handwashing posters posted, and floor to be free of hazards.

A first aid kit is kept in a cabinet in each classroom, LPAs observed first aid kit for necessary supplies. Carbon monoxide detectors and smoke detectors are present in the facility. Fire extinguishers have been serviced on 5/5/2023. Hazardous items including poisonous cleaning compounds were stored inaccessible to children.

Snack/lunch menus were reviewed. Food and snacks were reviewed for availability, quantity and appropriateness to children in care. At 1:20 PM, LPA observed food preparation areas for safety, cleanliness and proper equipment. LPAs observed a pantry well stocked and organized with canned goods and other snacks, refrigerator temperatures meeting Title 22 regulations, cabinets filled with adequate amount of plates, bowls, cups, and silverware that meet that needs of amount of children in care. Facility serves breakfast, lunch, and snack. Meals are prepared at a central kitchen and delivered to facility. Director states meal items are dispersed to the classrooms where food is served family style.

At 1:30 PM, LPAs observed outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. LPAs observed a large play structure with two slides and climbing area to be properly maintained with cushioning material below that expands through most of playground, there is also a step up area covered in turf grass where LPAs observed children's lounging area, crawl through play structure, and large plastic blocks available for children in good repair. LPAs observed a quiet reading area available for children in outdoor play area, and child appropriate outdoor tables and chairs available through out outdoor play area that were free of hazards and maintained.

Required shade and fencing were inspected. LPA White advised Director that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to their water bottles for outside play, LPAs observed a bin filled with children's water bottles at time of inspection. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. Personal Rights of children were observed by LPAs.

At 2:12 PM, Staff and Children’s Records were reviewed by LPAs. Criminal Record Clearances were reviewed for adults. CPR card for Site Supervisor Rosalia Nava expires 2/15/2025. Inspection of required forms made. LPA White spoke with Supervisor Nava concerning the four out of five staff members who need to update their Mandated Reporter Certification. Supervisor Nava states that proof of training will be placed in each file. Children's files were reviewed and in found to contain all department required forms.

Incidental Medical Services:
This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The following were discussed with the Director:
LPA White provided advisory notice inclusive of the department regulation regarding sign in/out procedures for parents to follow. Facility has documentation inside the sign in/out books asking parents to sign and date pick-up and drop off appropriately.

Rooms that are off-limits need to be made inaccessible during operating hours. Smoking is prohibited.

The Director was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care.

The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked and batteries replaced as needed. Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location.

LPA advised Director that all adults 18 years of age and older providing Care & Supervision and/or have continuous presence in the facility shall adhere to a criminal background clearance with the Department of Justice, FBI and Child Abuse Index Check.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today 11/17/2023.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
