25 | Licensing Program Analysts (LPA's) Roxana Lopez and Saul Valenzuela conducted an unannounced Case Management inspection due to incident that was reported to the Department on April 22nd, 2024, and April 30th, 2024. LPAs met with Principal Robin Hurdle who guided LPAs on a tour of the facility. Census was taken.
On April 22nd, 2024, an incident was self-reported to the Department via Email by the facility who reported a parent alleges a child's personal rights were violated.
On April 30th, 2024, an incident was self-reported to the Department via Email by the facility who reported a parent alleges a child's personal rights were violated.
Report for April 30th, 2024 was reported within the required 24 hours. The purpose of the inspection was to obtain additional information and documentation regarding the incident reported to the Department.
Report for April 22nd, 2024, was not reported within the required 24 hours.Although the incident was reported on 4/22/2024, documentation and interviews proof that the investigation occurred in the following days of the incident. Per Principal, she emailed the written report to the wrong emailed in January when the incident happened. LPA provided Principal with the correct email address and advise that reports must be receive in 24 hours and 7 days for written report. LPAs were unable to interview Teacher at this time further investigation is required.
Incident that occurred on 4/30/2024: child alleged that a teacher violated their personnel rights. LAUSD conducted their investigation and concluded that the findings were unfounded. LPA conducted interviews with staff and child and no disclosure was made regarding this incident. Page 1 of 2
