Licensing Program Analyst (LPA) Lilia Hernandez conducted an unannounced case management inspection due to incidences that occurred at the facility. LPA arrived at the facility at 8:30AM and met with Allison Speight, Principal, who guided LPA on a tour of the facility. There were 25 children in care and 11 staff present upon arrival.

The incident that occurred on 05/13/2024, was reported to the Department on 05/13/2024, via telephone. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence.

Information reported to the Department indicated that Staff #1 and Staff #3 observed Child #1 alone in the classroom without supervision.

LPA conducted interview with Principal, Staff #1 – Staff #3 and obtained written statements for this incident.

Staff #2 does not recall leaving a child behind in the classroom when taking the other children in care outdoors to play. Staff #1 stated that Child #1 disclosed Staff #2 left them in the classroom. Staff #3 confirmed that when they were walking back to the classroom, they saw Child #1 in the classroom and Child #1 disclosed they stayed in the classroom because their leg was hurting. Staff #3 escorted Child #1 to the outdoor play yard and return Child #1 to their classroom but did not mention the incident to Staff #2.

Based upon information received from the interviews conducted it was determined that Staff #2 was not providing adequate care and visual supervision when Child #1 was left alone in the classroom when the other children were taken to the outdoor play yard. This is an immediate risk to the health and safety of children in care.

The incident that occurred on 05/29/2024, was reported to the Department on 05/30/2024, via telephone. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence.

Information reported to the Department indicated that Staff #4 may or may not have violated the personal rights of Child #2 while in care.

LPA conducted interviews with Principal, Staff #4 and obtained copies of interviews conducted with Child #2 and Child #3.

Based upon information received from the interviews conducted it was determined that Staff #4 violated the personal rights of Child #2 when Child #3 observed Staff #4 pulled on Child#2 hoodie in order to get Child #2 to lay back down on their cot during nap time. This is an immediate risk to the health and safety of children in care.

The incident that occurred on 06/14/2024, was reported to the Department on 06/14/2024, via telephone. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence.

Information reported to the Department indicated that Staff #4 may or may not have violated the personal rights of Child #2 while in care.

LPA conducted interviews with Principal, Staff #4 and obtained written statements from Staff #5 and Staff #6 for this incident.

During an interview with Staff #4, Staff #4 disclosed that in reaction to Child#2 stating to Staff #4 “I hate you.” Staff #4 responded “I could hate you too.” Staff #4 disclosed that after the incident occurred, they realized that the incident could have been handle in a better manner.

Based upon information received from the interviews conducted it was determined that Staff #4 violated the personal rights of Child #2 when Staff #5 and Staff #6 observe Staff #4 disclosed that in reaction to Child#2 stating to Staff #4 “I hate you.” Staff #4 responded “I could hate you too.” This is an immediate risk to the health and safety of children in care.

The following deficiencies listed on the attached LIC 809D are being cited in accordance with California Code of Regulations Title 22.

A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement form must be maintained in each child’s file immediately upon receipt from parent.

A copy of the LIC 9224 - Acknowledgement of Receipt of Licensing Reports was provided during this visit.

The Notice of Site Visit was given and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative.
