On 02/13/2024 at 11:00 AM an Unannounced Annual/Required Inspection was conducted by Licensing Program Analysts (LPAs) Katrina Chicote and Claudia Kam. Facility is currently licensed for a capacity of 114. Licensing staff announced purpose of visit and initially met with Elizabeth Okoro, Principal/Facility Reprsentative (FR), who granted entry to facility and provided guided tour both indoors and outdoors. The program currently operates a 7:00 AM to 5:30 PM Monday thru Friday. LPAs observed required licensing postings on Bulletin Board by entry.

At the initial start of the site visit, there is a total census of 37 children and a total census of 13 staff present. The facility is made up of three classrooms (RM 1, RM 2, and RM 3). LPAs observed 12 preschoolers with four staff (S1, S2, S3, S4 with additional S5 on break) in RMs 1 (located upon entry), 14 preschoolers with four staff (S6, S7, S8, and S9) in RM 2 (located through double doors in RM 1), and 11 preschoolers with four staff (S10, S11, S12, and S13) in RM 3 which were playing outside upon arrival to facility .

Beginning at 11:10 AM, furniture and equipment were inspected for age appropriateness and good repair in each classroom. LPAs observed surface of classrooms to be safe and clean, and child size tables and chairs available for children well maintained. LPAs observed learning stations for dramatic play, math, and science with toys well maintained free of loose and sharp parts and a library area available for quiet play. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings. Mats/cots were inspected. Per FR, linens are provided by the District and washed weekly by a service. Availability of drinking water was reviewed. LPAs observed personal water bottles available for children and drinking fountains available in each classroom. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, general area safety and sanitation. LPAs observed three bathrooms available, Bathroom 1: three toilets, two sinks, and a walk up diaper changing table within arm's reach of a sink, Bathroom 2: two toilets and two sinks, Bathroom 3: one toilet, two urinals, and two sinks available.

Snack/lunch menus were reviewed and observed posted at time of inspection. All meals and snacks are provided by the Los Angeles Unified School District. Children are provided Breakfast, Lunch, and PM Snack. LPAs observed staff disposing of food in trash bags that are tied and disposed of immediately.

A first aid kit is kept on a mobile cart in hallway. At 11:15 AM, carbon monoxide detector was tested and observed operable at time of inspection. LPAs observed smoke detectors and fire extinguishers present in the facility. LPAs observed disaster drill log available, last drill conducted on 02/09/2024. Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. At 11:40 AM, LPAs observed medicines for C1 and C2 to be stored in closet by Administrator which is inaccessible to children. LPAs discussed Title 22 Regulation requirements for storage of medication at time of inspection. Per FR, isolation area for sick children is in corner of each classroom.

Beginning at 11:30 AM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. LPAs observed benches and seating areas under shaded area. LPAs observed a large play structure with climbing equipment, steps, and at least two slides has cushioning material to absorb fall that is well maintained. LPAs observed bicycles with concrete bike path maintained and plastic playhouse, and other toys available for children to use outside that are free of loose and sharp parts at time of inspection. Required shade and fencing were inspected. LPAs advised that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to their water bottles and two drinking fountains for outside play. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. LPAs observed all children present signed in at time of inspection. Personal Rights of children were observed by LPAs.

Beginning at 12:00 PM, staff and children’s records were reviewed. Children’s Records were reviewed for completeness. Inspection of required forms was made and documented on the LIC 857. The review of Staff records was documented on the LIC 859.

Staff files reviewed did have proof of the AB 1207 Mandated Reporter Training certificate on file. LPAs observed Pediatric First Aid/CPR expires on 11/2025 for FR. LPAs did not review confidential files at time of inspection which includes LIC 503. LPAs reviewed education verification at time of inspection. All staff and employees have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Los Angeles Unified School District.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES:
LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

Licensee (or facility representative) was reminded that all adults 18 and over in the facility, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed facility. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today only advisories given.

A notice of site visit was given and must remain posted for 30 days.
