On 05/15/2024 at 11:30 AM an unannounced Annual/Random Inspection was conducted by Licensing Program Analyst (LPA) Katrina Chicote. LPA announced purpose of visit and initially met with Facility Representative (FR), Humberto Benitez, Principal. Facility is currently licensed for a capacity of 166. Per FR they currently have 122 enrolled. The program currently operates Monday thru Friday from 7:00 AM to 5:30 PM. LPA was taken on a guided tour of the facility of both indoors and outdoors. LPA provided LIC 125 to FR. Required postings were observed on bulletin board in Office.

The facility is made up of five classrooms, Rooms 1, 2, and 3 are all in the same building and Rooms 4 and 5 are detached bungalow. At the initial start of the site visit, LPA observed Rm-1 (PCC) to have 19 children and four staff (S1, S2, S3, and S4), Rm-2 to have 18 children with three staff (S4, S5, and S6), Rm-3A to have 13 children with two staff (S7 and S8), Rm-3B to have 16 children with three staff (S9, S10, and S11), Rm-4 to have 17 children with four staff (S12, S13, S14, and S15), and Rm-5 to have 14 children with three staff (S16, S17, and S19). Census total was 97 children and 20 staff.

Beginning at 11:32 AM, furniture and equipment were inspected for age appropriateness and good repair in each classroom. LPA observed surface of classrooms to be safe and clean and child size tables and chairs available for children well maintained. LPA observed learning stations for dramatic play, math, and science with toys well maintained free of loose and sharp parts and a library area available for quiet play. Telephone service, heating, lighting and ventilation were evaluated. LPA observed children eating lunch in classrooms at time of inspection and transitioning to nap time. Per staff, food items are discarded in outside garbage cans which LPA observed to have tight fitting lids at time of inspection. There were cubbies observed as storage for children's belongings. Cots were inspected. LPA observed cots laid out in classrooms that did not hinder walkways and entrances/exits at time of inspection. Per FR, linens are provided by the District through a laundry service and washed weekly every Wednesday. LPA did observe laundry service at time of inspection providing fresh sheets and picking up used sheets. Availability of drinking water was reviewed.

LPA observed drinking fountains in each classroom and children's personal water bottles. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. LPA observed three bathrooms available for Rms 1, 2, and 3 that have five toilets, two urinals, and six sinks available. LPA observed a shared bathroom available for Rms 4 and 5 that have four toilets and two sinks. LPA observed each classroom to have additional sink.

Beginning at 11:41 AM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. Required shade and fencing were inspected. LPA observed two outdoor play areas available for children, both areas have an age appropriate play structure with cushioning material below to absorb fall that is well maintained at time of inspection. LPA observed age appropriate seating available for children in various areas of outdoor play area. LPA observed bicycles available for children with a concrete bike path that has been well maintained. LPA advised that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to drinking fountains during outside play as well as their personal water bottles. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

LPA observed a first aid kit is kept in a backpack in each classroom. At 12:15 PM, LPA tested carbon monoxide detector and observed to be in operable condition at time of inspection. LPA observed smoke detectors and fire extinguishers are present in the facility. Sick/isolation area is in Office. Per FR, hazardous items including poisonous cleaning compounds are not stored at the facility per LAUSD requirements. Facility is cleaned by janitorial service daily.

Snack/lunch menus were reviewed and observed posted. Food and snacks were reviewed for availability, quantity and appropriateness to children in care. Per FR, food is not prepared at facility and is delivered by nearby elementary school campus.

Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. LPA observed all children present signed in at time of inspection. Personal Rights of children were observed by LPA.

Beginning 12:30 PM, staff and children’s records were reviewed for completeness. Inspection of required forms was made and documented on the LIC 857 for children and the LIC 859 for staff. CPR card expires on 06/2025 for S12. 6 of 6 staff files reviewed did have proof of current AB 1207 Mandated Reporter Training certificate on file. LIC forms were discussed and provided. All staff and employees have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Los Angeles Unified School District (LAUSD).

The following were discussed:
The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and Carbon Monoxide detectors should be checked and batteries replaced as needed.

LPA advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation, this includes any changes as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location.

LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

Facility Representative was reminded that all adults 18 and over in the facility, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed facility. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES:
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

LPA referred Facility Representative to the Department website for lead: Lead Toxicity Prevention and Water Testing Information.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records.

MyChildCarePlan.org – Centers and Family Child Care Homes Facility Representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today, 05/15/2024.

A notice of site visit was given and must remain posted for 30 days.
