On 05/10/2024 at 10:30 AM Licensing Program Analysts (LPAs) Katrina Chicote and Andrea Carter conducted an Unannounced Case Management - Incident inspection to follow up on an incident that was reported to The Department on 05/06/2024. Upon arrival, LPAs met with Facility Representative, Charletha Robinson, Principal. Census was taken.

On 05/06/2024, an incident was reported to The Department where a parent reported to staff that her child's personal rights were violated at the facility. The facility reported this incident to The Department within the required 24 hours and submitted a written report within seven days, complying with Title 22 Regulation reporting requirements.

During this inspection LPAs interviewed Principal who was notified of incident. FR states that they are following LAUSD protocol on handling these types of incidents which included reporting to LAPD and CAHL. At this time, FR is still waiting for clearance from LAPD to begin internal investigation. FR states she will provide documentation in regards to incident when it becomes available.

Based on the information obtained on today's visit there is not sufficient information to make any determinations, and Department may return for another visit.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today, 05/10/2024.

A notice of site visit was given and must remain posted for 30 days.

Exit interview was conducted and report was reviewed with the Facility Representative, Charletha Robinson.
