25 | Licensing Program Analysts (LPAs) Lilia Hernandez and Roberto Luque Avila conducted an unannounced case management inspection due to an incident that occurred at the facility. LPAs arrived at the facility at 8:11AM and met with Ericka Griffen-Champ, Principal/Facility Representative. LPAs singularly conducted a tour of the facility. There were 38 children and 11 staff present upon arrival.
The incident that occurred on 06/07/2024, was reported to the Department on 06/10/2024, via email. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence.
Information reported to the Department indicated that Staff #4 may or may not have violated the personal rights of Child #4.
LPA Hernandez conducted interviews regarding this incident with Principal, Staff #4, and Staff #8. LPA Hernandez made attempts to interview Child #4. Child #4 is still enrolled at the facility but has not been present during the inspections.
LPA Hernandez obtained rosters and other pertinent information. Based on the information received during the interviews, it was determined that Child #4 personal rights were not violated.
There were no deficiencies cited during today’s inspection.
The Notice of Site Visit was given and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative.
