Licensing Program Analyst (LPA), Keyona Scott, conducted an unannounced annual required inspection to the Child Care Center on 10/06/2022. LPA conducted facility risk assessment in the morning with Director via telephone. LPA met with Director, Marcella McKnight at 10:43 AM prior to conducting annual required inspection for the infant program (facility# 197408649). LPA was guided on a tour inside and outside of the facility. LPA observed proper teacher to children ratios. At approximately 11:03 AM, LPA observed the preschoolers outside in the outdoor play yard. LPA observed the following: Blue Bamboo classroom (2 to 3 year olds) eleven (11) children to four (4) teachers; Red Sunflowers (3 year olds) fifteen (15) children to three (3) teachers and Yellow Sunflowers (4 year olds) thirteen (13) children to four (4) teachers. All Adults present, working and or volunteering at the Center have a criminal record clearance or exemption.

This is a preschool program that serves children ages two (2) years old to four (4) years old. The Child Development Center is located on the premises of Los Angeles Southwest College. There is also an active infant center (facility# 197408649) located on the premises and a school-age program (facility# 191805372) that is currently inactive. The preschool center operation hours are Monday through Thursday 7:30 AM to 3:00 PM and Friday 7:30 AM to 12:00 PM.

PHYSICAL PLANT
All areas identified on the Facility Sketch were inspected. The preschool program consists of three (3) classrooms; Blue Bamboo (2 to 3 year olds), Red Sunflowers (3 year olds) and Yellow Sunflowers (4 year olds). LPA observed the following in each of the three preschool classrooms: cubbies with each child's name at the entrance of the classrooms, outlet covers over the electrical outlets, table and chair sets, kitchen play sets, baby dolls, books, blocks and other age-appropriate playthings. LPA also observed napping cots in each classroom.

The facility was kept clean, neat and orderly. LPA observed two (2) toilets, one (1) urinal and three (3) sinks, in each of the preschool classrooms. LPA observed toilets, urinal and sinks to be safe, sanitary and functioning. All poisons and hazardous items are stored and inaccessible to children in care. All floors are clean and safe. LPA observed the classrooms and all kitchen areas and/or food preparation areas clean and free of litter, rubbish, rodents and/or any other vermin, flies, gnats and other insects. Trash cans used to discard food have tight fitting lids.

Outdoor play area is located adjacent to each classroom. Blue Bamboo (2 to 3 year olds) have their own outdoor play area that is separate from the Red Sunflowers (3 year olds) and Yellow Sunflowers (4 year olds). The Red Sunflowers and Yellow Sunflowers share the same outdoor play area. Outdoor play equipment is in safe condition and is free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. LPA observed age-appropriate play structures and equipment in the outdoor play area. There is adequate shade area in each outdoor play area. Drinking water is readily available both indoors and outdoors.

FACILITY RECORDS

There is at least one person present at facility that is trained in Pediatric CPR and First Aid. Per file review, LPA observed completion of Mandated Reporter training certificate in staff's files reviewed.



LPA reviewed two staff files of the following personnel records during today's inspection: Staff Qualifications; Proof of immunization of measles (MMR), pertussis (Tdap) and influenza; Current Pediatric CPR and First Aid Certification; TB clearance or risk assessment; LIC 503 (Health Screening Report); LIC 508 (Criminal Record Statement); LIC 9108 (Statement Acknowledging Requirement to Report Child Abuse); Mandated Reporter Training Certificate; LIC 9052 (Employee Rights)

LPA reviewed three (3) children's files of the following children's records: LIC 613A (Personal Rights); Admission Agreement; LIC 701 (Physician's Report); LIC 995 (Notification of Parents' Rights); LIC 627 (Consent for Emergency Medical Treatment); Immunization Record

The following was thoroughly discussed:

Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

The facility was operating in substantial compliance during today’s inspection on 10/06/2022. Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, no deficiencies cited.

A notice of site visit was given and must remain posted for 30 days.

Exit interview conducted and report was reviewed with Director, Marcella McKnight.
