On 06/20/2024 at 9:15AM an unannounced Annual/Random Inspection was conducted by Licensing Program Analyst (LPA) Katrina Chicote. LPA announced purpose of visit and initially met with Facility Representative (FR), Clarissa Andrews, Principal. Facility is currently licensed for a capacity of 112, per FR they currently have 95 enrolled. The program currently operates Monday thru Friday from 7:00 AM to 5:30 PM. LPA singularly toured facility and partial tour was provided by FR. Required postings were observed and discussed. LIC 125 was provided.

The facility is made up of three classrooms, RM-1, RM-2/2B, and RM-3. Upon arrival to the facility, LPA observed all classrooms using outdoor play area. LPA observed 34 students with nine staff members.

Beginning at 9:20 AM, furniture and equipment were inspected for age appropriateness and good repair in each classroom. LPA observed surface of classrooms to be safe and clean and child size tables and chairs available for children well maintained. LPA observed learning stations for dramatic play, math, and science with toys well maintained free of loose and sharp parts and a library area with soft seating available for quiet play. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings located in hallways. Cots were inspected and observed stored in cabinets in classrooms. Per FR, linens are provided by LAUSD and washed weekly through laundry service. Availability of drinking water was reviewed. LPA observed operable water fountains in each classroom. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. LPA observed three shared bathrooms available across from classrooms: Bathroom 1 (Girls) had three toilets and two sinks, Bathroom 2 (Unisex) had three toilets and two sinks, and Bathroom 3 (Boys) had one toilet, two urinals, and two sinks. LPA observed two additional bathrooms available in RM-3 that each had one toilet and one sink. LPA observed additional sinks in each classroom.

A first aid kit is kept in a backpack in each classroom. At 10:00 AM, LPA tested carbon monoxide detector and observed to be in operable condition at time of inspection. LPA observed smoke detectors and fire

extinguishers are present at the facility. Disaster drill log was available and last drill conducted was on 05/23/2024. Isolation area for sick children is either in Principal's Office or outside. Per FR, only LAUSD approved disinfectants are present at the facility and no additional cleaning compounds are present. LPA observed LAUSD approved disinfectants stored in cabinet that had operable child safety lock at time of inspection. LPA observed medicines for four children stored centrally in Teacher Only area that is locked at time of inspection. LPA reviewed storage of medication and required documents that comply with Title 22 Regulation requirement.

Snack/lunch menus were reviewed and observed posted. Per FR, food is delivered to facility by LAUSD and distributed at the facility. Food preparation areas were toured for safety, cleanliness and proper equipment. Facility provides AM Snack, Lunch, and PM Snack.

Beginning at 9:38 AM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. Required shade and fencing were inspected. LPA observed a variety of seating available through out outdoor play yard in areas that provide shade. LPA observed large outdoor play area with multiple sections labeled for: Dramatic Play, Library, Art, and Nature Exploring. LPA observed a variety of age appropriate toys made for outdoor play available for children well maintained free of loose and sharp parts. LPA observed multi-level play structure with slide and climbing equipment with cushioning material below to absorb fall well maintained as well as other surfaces maintained safe for activities planned such as bicycle path and grass/dirt for garden area. LPA observed one play structure with a surface below that was not maintained that FR states is not currently in use due to it being in the process of being updated. LPA advised FR that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. LPA observed that children have access to drinking fountains during outside play as well as their personal water bottles. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed.

Teacher-child ratios were observed and staff names recorded. All staff and employees have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Los Angeles Unified School District. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed and LPA observed all children present signed in at time of inspection. Personal Rights of children were observed by LPA at time of inspection.

Beginning at 11:15 AM, staff and children’s records were reviewed. Inspection of required forms was made and documented on the LIC 857 for children and the LIC 859 for staff. Confidential forms were not reviewed during today's inspection as they are maintained in LAUSD District Office. CPR card expires on 03/2026 for Staff #3. 6 of 6 staff present did have proof of the AB 1207 Mandated Reporter Training certificate on file.

The following were discussed:
LPA advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation, this includes any changes as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location.

LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES:
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records.

MyChildCarePlan.org – Centers and Family Child Care Homes FR was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

The facility was found in compliance per Title 22 regulations no deficiencies cited today, 06/20/2024.

A notice of site visit was given and must remain posted for 30 days.
