On 10/03/2024 at 12:35 PM Licensing Program Analysts (LPAs) Claudia Kam and Joshua Ortega conducted an Unannounced Case Management inspection to follow up on several incidents that were reported to The Department. Upon arrival, LPAs met with Facility Representative, Clarissa Andrews, Principal, who provided LPAs a tour of the facility. Census was taken. There are 23 children and 10 staff present at time of visit.

On 09/16/24, 6/18/24, and 5/07/24 Facility Representative reported incidents regarding allegations made towards staff involving the violation of children's personal rights. The facility reported these incidents to the Department within the required 24 hours and submitted a written report within seven days per Title Regulation requirements.

During this inspection, LPAs made observations, reviewed pertinent documents. Interviews with staff and children conducted on 6/20/24 by LPA Katrina Chicote and on 9/26/24 and 10/03/2024 by LPA Claudia Kam. Facility Representative followed LAUSD protocols regarding these types of incidents, reporting to LAPD Child Abuse Unit, reporting Operations Coordinator and writing an ISTAR and SCAR report. Facility Representative was instructed by LAPD to handle administratively. Facility Representative states that they conducted their own internal investigations regarding the reported incidents which includes interviews of parents and children and obtaining written statements from Staff. The internal investigations have been completed and all were supported. Based on interviews conducted the allegations regarding personal rights are substantiated for allegations reported as unusual incidents on 5/7/24, 6/18/24 and 9/16/24. Interviews confirmed violation of personal rights of children in care. Interviews with staff A2, A3, A4, A6, and C1 confirmed observing staff whispering in a menacing manor and yelling at children which is a violation of personal rights.
Unusual Incident reported 9/16/24 has been substantiated. Incident regarding denial of rights to use the bathroom was observed involving staff 1 was observed by staff 2 and staff 4. Incident of denial of food by staff 2 was observed by staff 4.

Facilty Represenative will have staff conduct a training on childrens rights and systems of support advisor will conduct a customized training program for personal development. Representive will submit a staff sign in sheet as verification of completion and staff attendance by 11/30/24.

Based on CCLD investigation, the Licensee did not meet the above requirement by not preventing intimidation of children in care on multiple occasions witnessed by multiple parties and denial of daily living functions. This poses an immediate health, safety, and personal rights risk to children in care.

Based on this information the following deficiencies on the LIC 809 D are being cited today 10/3/2024.

LPA Claudia Kam informed facility representative that this report dated 10/3/24 documents 1 Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.



Also, LPA Claudia Kam informed the facility representative to provide a copy of this licensing report dated 10/03/2024 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A notice of site visit was given and must remain posted for 30 days.
