On 03/22/2024 at 9:00 AM Licensing Program Analyst (LPA) Katrina Chicote conducted an Unannounced Case Management inspection to follow up on an incident that was reported to The Department on 02/12/2024. Upon arrival, LPA announced purpose of visit and met with Facility Representative, Amber McKenzie, who granted entry into facility. Principal was not available during today's inspection due to a Principal training. LPA singularly toured facility. Census was taken.

On 02/12/2024, an incident was reported to The Department in regards to an allegation of a violation of a child's personal rights. The facility reported this incident to the Department via email within 24 hours and submitted written report within seven days, per Title 22 Regulation requirements.

During this inspection LPA interviewed staff that incident was reported to and made observations of staff interactions in classroom. Per interviews, child's Authorized Representative reported allegations to staff which was immediately reported to Principal, LAPD, and Licensing. Per staff interviews, LAPD is currently doing it's own investigation which included interviewing child and child's Authorized Representatives. Per Staff, LAPD investigation is currently ongoing at this time. LPA observed child named in report as present at the facility and observed child's personal rights are met at time of inspection. LPA obtained and reviewed documentation which includes an email sent to staff and parents in regards to a removal of a staff due to an ongoing investigation being conducted by LAPD. LPA spoke to Principal, Vivian Beans, over the phone who stated that they would email Personnel Report, Facility Roster, and LAPD Police Report # to LPA as soon as training has concluded by end of today, 03/22/2024. Due to insufficient information being available at this time, a follow up visit will be required at a later date.

The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today.

A notice of site visit was given and must remain posted for 30 days.

Exit interview was conducted and report was reviewed with the Facility Representative, Amber McKenzie.
