25 | On 12/06/2023 at 1:40 PM Licensing Program Analyst (LPA) Katrina Chicote conducted an Unannounced Case Management - Incident inspection to follow up on an incident that was reported to The Department on 11/02/2023. Upon arrival, LPA announced purpose of visit and met with Gloria Machado, Principal. Census was taken.
On 11/02/2023, an incident was reported to The Department where a child made an allegation towards a staff member in regards to personal rights. The facility reported this incident to the Department within the required 24 hours and submitted written report within seven days.
During this inspection, LPA obtained pertinent documents and interviewed staff who all worked in the same classroom. Interviews with three staff stated they did not observe any inappropriate interactions between staff and children at the facility. LPA interviewed three children, who were all qualified, and they did not provide any corroborating information in regards to incident. All children provided consistent statements that they felt safe at the school. LPA reviewed documents provided confirming that District is conducting their own internal investigation in regards to incident, which at this time is still ongoing. Staff named on report is currently not present at the facility since allegation was made and LPA observation of facility confirm staff not present during inspection.
Based on information obtained during this inspection, a return visit may be required at a later date. Principal states she will keep Department up to date on status of District's investigation and provide finding once one becomes available.
The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today, 12/06/2023. A notice of site visit was given and must remain posted for 30 days.
Exit interview was conducted and report was reviewed with the Facility Representative, Gloria Machado.
