On 05/10/2024 at 2:30 pm Licensing Program Analysts (LPAs) Katrina Chicote and Andrea Carter conducted an Unannounced Case Management - Incident inspection to follow up on an incident that was reported to The Department on 04/12/2024. Upon arrival, LPAs announced purpose of visit and was granted into facility and met with Facility Representative (FR) Gloria Machado, Principal. Census was taken.

On 04/12/2024, an incident was reported to The Department where a parent made an allegation in regards to their child's personal rights being violated at the facility. The facility reported this incident to the Department within the required 24 hours and submitted written report within seven days.

During this inspection, LPAs reviewed pertinent documents and interviewed FR who followed LAUSD protocol and reported to CAHL and LAPD. LAPD instructed to conduct administratively, and FR states she did conduct internal investigation which included interviews of children and staff and there was no corroborating information provided in interviews. LPAs reviewed written statements provided which are consistent to FR statements. Per FR, teacher that child alleges hit her child is not child's classroom teacher and do not have any interactions with each other, written statements corroborate this information. FR also states that child denied the allegations. Per FR, child has not been attending the facility but currently still enrolled and they have been consistently checking in with the child's parent for an update. Per FR, they have been in communication with the parent earlier this week. FR states child may still return but after one week they will have to provide termination letter via mail if she does not return.

Based on information obtained during this inspection, there will be no deficiencies cited.
A notice of site visit was given and must remain posted for 30 days.
Exit interview was conducted and report was reviewed with the Facility Representative, Gloria Machado.
