25 | On 07/11/2024 at 9:20 AM Licensing Program Analysts (LPAs) Katrina Chicote and Peter Bishop conducted an Unannounced Case Management inspection to follow up on an incident that was reported to The Department on 06/24/2024. Upon arrival, LPAs announced purpose of inspection and met with Facility Representative (FR), Gloria Machado, Principal. Census was taken.
On 06/24/2024, an incident was reported to The Department where a child fell and suffered an injury requiring medical attention. The facility reported this incident to The Department within the required 24 hours and submitted a written report via email.
During this inspection LPAs were provided tour of outdoor play area where incident took place and interviewed Teacher who observed incident. LPAs observed outdoor play area surface to be made of concrete that has been well maintained for the activities planned, LPAs did not observe any trip or fall hazards around area where incident took place. Interviews state that there was a Teacher who observed child fall and they were able to administer first aid immediately which involved applying ice to the area. Teacher states child is very active and runs often and does have a tendency to lose balance, which has been discussed with child's parents. FR state that child's parent was called and they arrived to pick up child after an hour and they took child to doctor where it was found out that child suffered a fracture. Teacher stated that at time of incident child was crying but was able to move her arm freely. FR states that they have been in communication with child's parents and they were informed that child currently has a cast on and though FR states they could offer reasonable accommodations child's parent is choosing to keep them home until cast has been removed. FR states that she has had a meeting with staff after the incident to go over transitions and safety rules. Based on the information obtained and LPAs observation of the play area, LPAs determined there was adequate supervision and response provided the day of the incident as well as trainings provided to staff after incident occurred.
The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited today, 07/11/2024. A notice of site visit was given and must remain posted for 30 days.
Exit interview was conducted and report was reviewed with the Facility Representative, Gloria Machado.
