25 | On 3/5/25 at 2:40 PM Licensing Program Analysts (LPA) Claudia Kam conducted an Unannounced Case management Inspection to follow up on an incident that was reported to the Department. Upon arrival, LPA announced purpose of inspection and met with Facility Representative, Martha Aranas, who granted entry to facility. Census was taken.
On 2/12/25, an incident was reported to The Department where a child sustained an injury while in care. The facility reported this incident to the Department within the required 24 hours and submitted written report within seven days.
During this inspection LPA interviewed staff, observed indoor play area where incident took place. LPA was unable to observe child named in report as child had left for the day. Teacher who witnessed the incident informed Licensing that a child reported an injury in care but appeared to be an old cut. Child's hands were washed and injury examined. Child did not complain of pain, no signs of injury observed and remained in care the rest of the day. LPA observed classroom area to be free of hazards. Based on the information obtained and LPA observation, LPA determined there was adequate supervision.

A notice of site visit was given and must remain posted for 30 days.
Exit interview conducted and report was reviewed with the facility representative Gloria Machado.
