25 | Licensing Program Analyst (LPAs) Roxana Lopez and Priscilla Ochoa conducted an unannounced Case Management Incident inspection to follow up on 3 incidents that were reported to the Department. LPA's met with Principal Carolyn Macaranas who guided LPAs on a tour of the facility. Census was taken.
On December 5th, 2023, an incident was self reported to the Department via Email by the facility who reported that a child hit their head requiring medical attention.
On December 6th, 2023, an incident was self reported to the Department via Email by the facility who reported that Parent alleges child's personal rights were violated.
On June 4th, 2024, an incident was self reported to the Department via Email by the facility who reported that Parent alleges child's personal rights were violated.
All reports were reported within the required 24 hours. The purpose of the inspection was to obtain additional information regarding the incident reported to the Department.
During the inspection, LPAs conducted interviews with staff and obtained documentation. Regarding incident that occurred on December 5th, 2023- incident was observed by staff and first aid was provided. Child was transported to hospital- required 3 stiches and came back to facility with no restrictions.
Regarding incidents that occurred on December 6th, 2023 and June 4th, 2024 interviews were conducted and no disclosures were made.
For this inspection, the licensee is in compliance with California Code of Regulations Title 22. No deficiencies cited.A notice of site visit was given and must remain posted for 30 days.
