25 | Licensing Program Analyst (LPA) Lilia Hernandez conducted an unannounced case management inspection on 05/07/2024 to follow up on incidences that were reported to the department. LPA arrived at the facility at 9:45AM and met with Viken Kazarian, Principal who guided LPA on a tour of the facility. There were 55 children and 17 staff present upon arrival.
The incidences that occurred at the facility on 04/09/2024, were reported to the Department on 04/12/2024, via telephone. The facility made the report to the Department within the required 24 hours of occurrence.
Information reported to the Department indicated that Child #1 and Child#2 personal rights may or may not have been violated.
LPA conducted interviews with Staff #1 - Staff #8 and obtained rosters and other pertinent information. LPA also conducted observations of care and supervision of children in care. During interviews with staff, disclosures were made regarding conflict between staff, however, at not time were disclosures made where staff had concerns with the health and safety of children in care. Principal disclosed that all personnel are being provided with training's to ensure personnel are competent to provide the services necessary to meet the individual needs of children in care.
Based upon information received from the interviews conducted and observations made, it was determined that the personal rights of Child #1 and Child#2 were not violated.
There were no deficiencies cited during today’s inspection.
A notice of site visit was given and must remain posted for 30 days.
