25 | Licensing Program Analyst (LPA) Lilia Hernandez conducted an unannounced case management inspection on 10/04/2024 to follow up on an incident that was reported to the Department. LPA arrived at the facility at 1:00PM and met with Barry Rubenstein, Head Teacher, who guided LPA on a tour of the facility. There were 44 children and 11 staff present upon arrival.
The incident that occurred at the facility on 09/17/2024, was reported to the Department on 09/17/2024, via telephone. The facility made the report to the Department within the required 24 hours of occurrence.
Information reported to the Department indicated that Child #1 bit Child #2 and Staff #2 while in care.
During todays inspection, LPA obtained a copy of the facility roster. LPA also conducted an interview with Head Teacher who is the personnel responsible for the care of Child #1.
During the interview, Head Teacher disclosed that at no time was Child #1 and Child #2 left unsupervised. Head Teacher stated that the facility is currently working with Child #1's parents to obtain resources for Child #1. Further information is needed to ensure the facility is able to meet the needs of Child #1 while in care.
There were no deficiencies cited during today’s inspection.
A notice of site visit was given and must remain posted for 30 days.
