32 | All staff stated they were all trained on the procedures of toileting by viewing a video, reading a power point and signing an acknowledgment form stating the understood the procedure.
Per all staff, the procedure for assisting children with bathroom duties is to assist at the opening of the bathroom in plain view of the mirrors in the hallway and the classrooms. Most children do not need assistance, however if there is a need, one staff changes the child while another staff is present to observe and assist. Upon entering the hallway LPA observed Staff #2 (S2) change a child while another staff observed.
On 2/17/26 Child #1 (C1), who is potty trained and capable of using the restroom on their own, soiled their clothes during nap time. Upon awaking, they asked (S2) to assist with changing their clothes. Per S2 C1’s legs appeared slightly wet. C1 was wearing boots and was struggling to remove them, so S2 assisted with the boots. S2 provided clean clothes from the cubby box. When they saw C1 continuing to struggle, they asked if help was needed, and C1 nodded yes. S2 opened the underwear so C1 could place each foot inside. C1 also had difficulty getting a foot into the leggings. S2 asked again if help was needed, and C1 nodded yes. S2 held the foot openings so C1 could put each foot in one at a time. They did not perform any wiping because C1 is able to do that independently; C1 did not wipe because the clothing was wet. All assistance took place in the hallway, in an open and visible area.
All staff members interviewed did not see C1 being touched inappropriately, nor did C1 disclose they were touched inappropriately. All children interviewed stated they were not touched inappropriately.
Parent #1(P1) stated they are not comfortable with their child being wiped by anyone because the child is fully potty trained and do not need any assistance, however P1 does not want to remove C1 from their present class because they are very comfortable with S2.
Based upon information received from the interviews conducted and observations made, it was determined that the personal rights of Child #1 were not violated.
There were no deficiencies cited during today’s inspection.
Exit interview conducted and report was reviewed with Principal Lessly Mora. A Notice of Site Visit was given and must remain posted for 30 days.
