| Regarding the first incident reported on October 14th, 2024- The department received two calls on this date stating that a staff # 1 was observed twice violating the same child's personal rights at different times. LPA interviewed staff and children. LPA did obtained copies of staff declarations and Unusual Incident Reports submitted on this date. Per Principal Staff # 1 has been removed from facility indefinitely.
Interviews and disclosures of two staff members disclosed that child was treated aggressively by staff # 1, First incident child was grasp aggressively and was maintained held while in line and second incident observed child was picked up aggressively from the floor and sat unto a chair aggressively by staff # 1. During interviews a staff members disclosed that staff # 1's tone of voice is too loud towards the children and is strict. This is an immediate risk for the health and safety of children.
Based on the LPA’s observations and records reviews, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health and safety.
LPA Roxana Lopez informed facility representative Irene Santillan that this report dated 10/23/2024 document(s) 1 Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.
Also, LPA Roxana Lopez informed the licensee facility representative to provide a copy of this licensing report dated 10/23/2024 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative Irene Santillan------------------ pg. 2 of 2 -----------
