25 | Licensing Program Analyst (LPA) Mireya GarcĂa conducted an unannounced Case Management inspection due to incidents that were reported to the Department on December 7, 2022, December 12, 2022, and January 27, 2023. Due to COVID- 19 precautionary measures were taken, licensing staff present during inspection wore appropriate personal protective equipment. LPA met with Head Teacher Cindy Pulido who guided LPA on a tour of the facility. Principal, Scarlett Ramirez-Holguin later arrived during this inspection. Census was taken.
On December 7, 2022, one incident was self reported to the Department via Email by the facility who reported a child was injured while in care.
On December 12, 2022, one incident was self reported to the Department via Email by the facility who reported a parent alleges that a child's personal rights were violated while in care.
On January 27, 2023, one incident was self reported to the Department via Email by the facility who reported a child was injured while in care.
The purpose of the inspection was to obtain additional information regarding the incidents reported to the Department.
During the inspection, LPA Garcia conducted interviews with Principal, eight (8) staff and attempted to interview (1) day care child. LPA was unable to complete interviews on this date. Due to additional staff/children were not available for interview at this time, a follow up visit will be required at a later date in order to conduct further interviews and/or obtain additional documentation.
A notice of site visit was given and must remain posted for 30 days.
