25 | Licensing Program Analyst (LPA) Staicy Perry conducted an unannounced case management inspection to follow up with an incident that was reported to the department. LPA met with Alicia Lomeli, Director who guided LPA on a tour of the facility.
On February 27, 2025, it was reported to the department that on February 26, 2025, S#1 did not properly close the door in C#1’s classroom (room 2). C#1 pushed the door open and walked out of the classroom 2 to an area which leads to a small little walk way that doesn’t lead anywhere which is gated and no doors available to exit out anywhere. C#1 went out the walkway without anyone knowing until transition time to mealtime (lunch) the staff asked where is C#1, and primary caregiver got up and saw C#1 playing with toys in the walk way on the side of the facility. The incident was reported to the Department within the 24-hour period.
During today’s inspection, declarations were obtained, interviews were conducted, photos taken, video observed of incident and documentation was obtained and reviewed. C#1 did walk out of classroom through an exit door in the classroom that leads to a side walkway of the building. LPA Perry did observe video footage of incident. LPA Perry observed C#1 walk out of the classroom door unsupervised and unattended and was outside in the walkway for about 6 minutes and 11 seconds and without staff knowledge. Once S#3 asked S#2, “where is C#1?”, that’s when LPA observed S#2 walk towards the area where the door is at and via the windows observed the top of C#1’s head and S#2 opens classroom door to get C#1. LPA observed C#1 walk back into the classroom unharmed. LPA Perry did observe area where C#1 was left unattended and unsupervised. LPA took photos. LPA did not observe and hazardous / potentially dangerous materials in that area during inspection. Per staff no hazardous / potentially dangerous materials are kept there at any time. The walkway was observed and measured to be 82.5 feet long and 12.5 in width with a gate that is locked, also side gate that is over 5 feet tall and also has an additional latch buckle at the top of the side gate as well as a doorknob that is kept locked. Per S#5, P#1 was informed of incident on 02/26/2025. |
