On September 24, 2025, Licensing Program Analyst (LPA) Staicy Perry conducted an unannounced complaint investigation at the facility to deliver findings regarding the above allegation. Upon arrival, LPA Perry met with the facility Site Supervisor, Darlene Monarrez to whom the purpose of the visit was explained. Census was taken, and LPA observed the facility operating within Title 22 child-to-staff ratio requirements.

During the course of the investigation, LPA Perry conducted staff, child and parent interviews, attempted contact with reporting parties, reviewed facility records including children’s roster (LIC 9040), reviewed video footage, and gathered other pertinent documentation.

Reporting Party (RP) alleged that on July 15, 2025, Child #1 was punched in the head by Staff #3, resulting in a superficial scratch to the left temple. RP reported that when the child #1 was picked up from care, P#1 noticed sand in the child #1 hair, a bump on the child #1 head, and that the child #1 pulled

away when touched near the ear. Child #1 disclosed to RP that Staff #3, punched C#1 multiple times with for misbehavior. Additionally, RP Reported that Child #1 had been in care over a year and had been hit by children on several occasions. RP also alleged that on the same date (July 15, 2025), Child #1 stated a staff member had hit C#1.P#1 did disclose that facility did inform her that C#1 was hit with a wooden block outside.

During parent interviews conducted, it was disclosed that parent #4 child had also previously reported being hit by Staff #3. Parent #4 stated this was reported to administration, and the facility responded that the matter would be addressed.

During child interviews, no disclosures were made regarding staff hitting children.

During staff interviews, all staff interviewed denied ever seeing Staff #3 hit a child. Nor have they hit a child themselves.

LPA reviewed the facility’s unusual incident report filed in relation to Parent #4’s concerns as well as unusual incident report regarding the above allegation. Video footage was reviewed by the administrator as well as LPA Perry and LPA Ayala from July 15, 2025,the day that injury was reported. It showed Child #1 and Child #2 playing outside near the back of the play yard on July 15, 2025. Child #2 was observed throwing a wooden block connector at C #1 on the head near the ear, which may have caused the reported injury. The facility is associating C#1's injury with the incident for that day due to no other incidents observed during reviews of video footage. Staff #1 was present and gave Child #1 attention, It was also noted on the LIC 624 that S#1 touched C#1's head and asks what happened and C#1 responded that C#2 . C#1 was comforted and checked and no bump was observed by S#1. After the child stopped crying, no further action was documented. The administrator Haydee Portillo and Ana Gamino confirmed a full-day video review was completed; however, the facility will not provide a copy to the Department due to pending legal issues related to videos being released. LPA Perry did confirm via daily attendance logs that C#1 was present the day of July 15, 2025. LPA Perry also confirmed that staff #1and staff #3 was also present the day of July 15, 2025 per time sheet. It was noted that all the administrators observed that incident and did not observe other incidents with S#3.

LPA attempted to obtain a copy of the police report related to the allegation. On September 24, 2025, LPA Perry conducted a collateral visit at Hollenback Police Station Department located at 2111 E 1st street Los Angeles CA 90033 in attempt to obtain a copy of the police report. LPA spoke with the officer in the Hollenback Detectives unit who stated that at this time there is no report. He stated that it is currently assigned to a detective and is under review for investigation and that at this time there is no information to provide. LPA Perry was unable to obtain a copy of the police report at this time.

Multiple attempts were made to contact RP and a secondary RP. No return calls or additional information were received.

Based on observations, interviews, documentation reviewed, and video review information, there is not a preponderance of evidence to prove that Staff #3 hit Child #1. Therefore, the allegation is determined to be UNSUBSTANTIATED.An unsubstantiated finding means that although the allegation may have happened or may be valid, there is not a preponderance of the evidence to prove the violation occurred.

No deficiencies are being cited at this time.

An exit interview was conducted with the Site Supervisor, Darlene. Appeal Rights were provided, and a Notice of Site Visit (LIC 9213) was issued and must remain posted for 30 consecutive days.
