An unannounced subsequent inspection was conducted by Licensing Program Analyst (LPA) Roxana Lopez on 1/15/2026. The purpose of this inspection is to provide the findings of the complaint investigation which was received on 11/20/2025. LPA met with Principal David Zendejas, to whom the purpose of the inspection was announced. Per Principal, there are 80 children enolled. Census was taken.

Throughout the course of the investigation, interviews were conducted with staff and parents. LPA also reviewed and obtained copies of children’s rosters, pictures and other documentation.

Per initial complaint report, the complainant reported that room # 2 was full of clutter and unsafe for children. Per complainant the room had stacked shelves and other furniture in the room with 2 classrooms combined as room # 3 was under construction. Complainant was not available for interview.

In regards to allegation staff did not ensure room was free of cluttered: Staff # 1, 4, 5, 7 and 8 corroborated that furniture from classroom undergoing construction was stored in room # 2, while the children were present.

Additionally, during initial inspection conducted by LPA Priscilla Ochoa- LPA observed that room # 2 was used to store furniture from room # 3 while being used by children. LPA Ochoa also observed construction materials in the outdoor area- with only a yellow tape making area inaccessible for children. Per Principal, in charge on that date they will be ordering storage containers to store the furniture from classrooms undergoing construction.

LPA Lopez conducted interviews with parents. 2 out 5 Parent’s interviewed stated that they were not informed of construction ahead of time and did observed stacked furniture in room # 2. Parent’s 1-5 corroborated that they were not informed on where items for classrooms undergoing construction were going to be stored or where children will be when their classroom is undergoing construction. Per parent’s 1-5 they were informed during drop off where to take their child.

This agency has investigated the complaint alleging Staff did not ensure room was free of cluttered. Based on LPAs observations and interviews which were conducted and record reviews, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. The following deficiency listed on the attached LIC 9099D (deficiency page) is being cited in accordance with California Code of Regulations Title 22. Deficiency that is being cited need to be cleared to protect the children’s health and safety.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Principal David Zendejas.

During the course of the investigation storage bins were delivered and items are stored in the bins in the parking lot. Principal will send a written plan on how the facility will remain in compliance and safe for the children in care for the raminder of construction to LPA by POC due date of 1/23/26.
