Licensing Program Analyst (LPA) Roxana Lopez conducted an unannounced case management inspection due to incidents that occurred on July 7th, 2023, December 15th, 2023, March 18th, 2024 and July 23rd, 2024. LPA met with Principal, Barbara Walker who guided LPA on a tour of the facility.

LPA Lopez conducted interviews and obtained documentation during this visit.

Incident that occurred on July 7th, 2023 was reported on November 11th, 2023 which is not within the 24 hours hours of occurrence.

The facility reported the other three Unusual Incidents to the Department within the required 24 hours of occurrence.

Regarding incident # 1 and # 2 facility reported that same parent disclosed that their child (#1) alleged being inappropriately touched by 2 different children. Interviews were conducted with staff and children. No disclosures were made regarding incident # 1. Regarding the second incident child # 2 disclosed that they were hit by Child # 1 on the face and that they hit back, hitting child # 1 on the " no no square" and they both got "in trouble." Staff interviews disclosed that they spoke to the children about respecting their bodies and personal space.



Regarding incident # 3 facility reported that parent alleges that their child's personal rights were violated- as child disclosed that Staff # 4 would grab their hand harsh during writing assignments. Facility conducted interviews with children who corroborated the allegation is true as it happened to them or they witnessed it. Child # 4 corroborated that they have witnessed staff # 4 violating children's personal rights including child named on incident. No disclosures were made from staff.

Regarding incident # 4 facility reported that Child # 5 ran out of the playground into the parking lot. During interviews staff disclosed that the class had just come outside and were lined up. Child # 5 got out of line and opened the playground gate- teacher observed the child open the gate. Teacher aide went towards child and asked child to come back inside but child took off running down the ramp and pushed opened gate leading to parking lot. Teacher aide ran behind child and was asking child to stop- there was a parent at the end of the ramp that stood in front of the door to prevent child from exiting the premises. Child was brought back into the facility. Child # 5 disclosed that they opened the door by themselves and did not listen to staff when they asked them to stop. Per child staff was following them at all times and brought them back into the facility. LPA did observed a security lock on the playground door on this date.

Based on the LPA’s observations, interviews and records reviews, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health and safety.

A notice of site visit was given and must remain posted for 30 days.

Per Principal they will go through staff relations to request for Staff # 4 to be removed from the school- make sure all documentations and trainings are provided in the language necessary. Principal will submit proof of meeting agenda with signature of staff present.
