Licensing Program Analyst (LPA) Melissa Zaragoza conducted an unannounced complaint inspection to the above facility on 05/26/2026. LPA arrived at the facility at 9:30AM and met with the principal, Lilia Hernandez. The principal guided the analyst on a tour of the facility. There were 97 children and 20 staff present at the facility.
During the investigation LPA interviewed staff, interviewed parents, obtained a copy of the children's roster, and obtained copies of supporting documents.

Information provided by the Reporting Party (RP) indicates that staff does not ensure incident reports are accurate, and staff do not ensure playground equipment is safe for children. Per RP, staff did accurately report an injury that Child #1 sustained.

Per Principal, when any child sustains an injury parents are notified and an incident report is provided and parents are contacted. Per Principal, the playground equipment is in a safe condition, and the district placed play equipment that is part of the Nature Explore program prior to Principal working at the facility. Per Principal, they commenced in November of 2025.

While interviewing staff, staff disclosed that parents are always notified via a phone call or an incident report when a child sustains an injury. Per Staff #2 (S2), the incident that occurred with C1, was accurately reported to C1s guardian/authorized representative. Per staff interviewed, staff disclosed that the district transformed the outdoor playground in 2023 as a Nature Explore program to create an outdoor learning environment.

Parents interviewed made no disclosure regarding the allegations. P#2, stated that they always get a notified via a phone call and incident reports by that staff and are happy with the staff and the care provided.

Based on observations made during the LPA’s inspections, the outdoor play area was observed to be appropriately maintained and safe for children. LPA did not observe any broken equipment, sharp objects, exposed hazards, or unsafe playground structures during the visits.

Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with the Principal, Lilia Hernandez, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
