25 | Licensing Program Analyst (LPA) Lilia Hernandez conducted an unannounced case management inspection due to a self reported incident that occurred at the facility on 05/06/2024. LPA arrived at the facility at 1:20PM and met with Nina Jones, Designee, who guided LPA on a tour of the facility. There were 51 children and 13 staff present upon arrival. LPA was later met by Erika Mancia, Principal.
The incident that occurred on 05/06/2024, was reported to the Department on 05/07/2024 via telephone. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence.
Information reported to the Department indicated that Staff #1 may or may not have violated the personal rights of Child #1 while in care.
LPA previously conducted an interview with Principal, and obtained written statements from staff. LPA attempted to reach parent of Child #1 for clarification on what Child #1 had reported to the parent. LPA was unable to obtain information to determine if Child #1's personal rights were violated.
During todays visit, Principal disclosed that the Parent of Child #1 did not report any further information regarding the incident and Child #1 remained in care until 08/24/2024.
Based upon information received from the interviews and written statements, there were no deficiencies cited during today’s inspection.
The Notice of Site Visit was given and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative.
