Per Staff (S1) interview, Child (C1) was in the classroom playing on the rug with other children. Per S1, children in the class were letting them know, C1 was hitting other children. Per S1, they redirected C1 and instructed them to keep their hands to themselves. Per S1, they were standing in front of the children, reminding C1 not to throw toys. Per S1, while they stood in front of the children, C1 fell back, and hit their head on a toy. Per S1, they stated the observed C1 stand up after the fall. Per S1, they denied pushing C1, or putting their hands on them.
Based on children’s interviews, C1 stated that S1 hurt their head by accident, but also stated they hurt their own back by accident. Per interview with C2, when they have a hard day at school they receive, “sad faces”, as a form of discipline, from teachers. Per parent interviews, parents are content with the staff and seemed to have shared positive feedback.
During LPAs visit LPA did not observe staff physically handling children in an inappropriate manner. During observations, LPA did not witness any staff physically harming children or using inappropriate physical discipline or interactions with children in care. An unusual incident report regarding this incident was sent via email to the department and received on 02/27/2026.
Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is unsubstantiated,
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with the principal, Lilia Hernandez, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
