25 | Licensing Program Analyst (LPA) Veronica Martinez Garza conducted an unannounced Case Management- Incident inspection at the above facility on 10/12/23 at 10:15 a.m. A COVID-19 risk assessment was conducted prior to entering the facility. Per Office Manager, there was 1 confirmed case of COVID. LPA wore a mask during this inspection. LPA met with Principal Josefina Navarro who stepped away to attend a meeting and met with Office Manager Diana Bejarano, who guided LPA on a tour of the facility. Census was taken.
The purpose of this inspection is to follow up on an unusual incident report that occurred on 07/10/23 and was reported to the Department by the facility within the required 24 hours of occurrence.
Information reported to the Department indicated that Staff #1 violated Child #1’s personal rights.
During this investigation, LPA interviewed, Staff 1 (S1-3), Child 1 (C1), and obtained a copy of facility roster. LPA obtained LIC855 written Declarations from S1-S2.
LPA was unable to complete interviews on this date. Due to additional staff/children were not available at this time, a follow up visit will be required at a later date.
At this time, there were no deficiencies cited during today’s inspection.
The Notice of Site Visit (LIC 9213) must remain posted for 30 days during the hours of operation after each site visit by a licensing representative, a civil penalty of $100 can be assessed.
Exit interview conducted and report was reviewed with facility representative, Principal Josefina Navarro.
