On 1/31/2025 at 9:15 a.m., Licensing Program Analyst (LPA) Saul Valenzuela conducted an unannounced required inspection. LPA met with Principal, Josefina Navarro, who guided the LPA on a tour of the facility. This is a LAUSD preschool program located on the Northeast corner of Murchison Street Elementary School. This program is licensed for 148 children which operates Monday – Friday from 7:00 AM – 5:30 PM. Per the Principal there are 70 children enrolled.

All areas identified on the Facility Sketch were inspected. The following staff was present during this inspection: Room 1: empty due to low enrollment; Room 2: Staff #1, Staff #2, Staff #3, Staff #4 and Staff #5 with 7 children; Room 3: Staff #6, Staff #7, and Staff #8 with 13 children; Room 4: Staff #9, Staff #10, and Staff #11 with 11 children; Room 5: Staff #12, Staff #13, and Staff #14 with 12 children; Room Preschool Collaborative Classroom (PCC): Staff #15, Staff #16, Staff #17, Staff #18, and Staff #19 with 13 children. Teacher-child ratios were observed to be in accordance with Title 22 Regulations. During the time of the inspection, children in the PCC classroom were outside for outdoor play time.. All children were observed to be under supervision, including visual supervision, of a teacher always. The licensed facility is within the conditions, limitations, and capacity specified on the license.

LPA observed the facility to be clean, sanitary and in good repair. Furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. All toilets and washing facilities are in safe and sanitary operating conditions. ---------------------Page 1 of 5------------------------------

All materials and surfaces accessible to children are toxic free. Currently, the lobby is used as an isolation area. There are chairs and mats in the lobby. Parents are contacted immediately when children are determined to be ill, and staff are ensuring that children with obvious symptoms of illness are not being accepted.

The facility provides breakfast, lunch, and snack. Menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. Per Principal meals are dropped of daily by the cafeteria from Murchison Street Elementary School. All food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. All food is protected from contamination, and LPA inspected that any contaminated food is discarded immediately. There is drinking water available in the classroom via individual water bottles and disposable water bottles- which are taken outside for children to drink. LPA observed trash cans used for solid waste do have a tight-fitting lid.

At 9:44 a.m., LPA Valenzuela observed 2 cleaning solutions spray bottles labeled "Keep out of Reach of Children" located under the sink in Classroom #3. LPA discussed with Principal that the spray bottles must be stored in an area inaccessible to children. Per Principal, they instructed the custodian to place the spray bottles in the cleaning closet that is locked with a key. Per Principal that there are no poisons stored at the facility.

At 10:00 a.m. LPA observed that there are smoke and carbon detectors in the facility located in the lobby. The last Earthquake and Disaster Drill was conducted on 1/31/2025. LPA observed sleeping arrangements in form of mats.

Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. There is adequate shade in the play yard.

Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day.

There is at least one person trained in CPR and Pediatric First Aid present during this inspection. All staff have received an active criminal record clearance as a condition of their employment with the Los Angeles Unified School District.

LPA did not review complete staff files during the inspection due to the staff files being stored at the administrative office. LPA will review staff files at a later time and date. If staff files were to be incomplete during the time and date that LPA reviews files, a case management inspection will occur at the facility to reflect the incomplete files.

Children’s Records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file.

AB1207 Mandated Child Abuse Reporting – On or before March 30, 2018 any person who works in a child care facility shall complete the training and renew the training every 2 years. Website provided: https://www.mandatedreporterca.com/training/child-care-providers.



LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. For IMS information see PIN 22-02-CCP. At 10:15 a.m. LPA Valenzuela observed that Child #3 medication expired on 9/30/2024. LPA observed that that medication box states that "do not use after 3/14/2024". LPA confirmed that medication was not used on Child #3 since medication expired. Per Principal, they will contact Child #3 parents to supply a new medication and will discard or give back the medication to parent.

CCC COMPLETED SAMPLING BUT THE TEST RESULTS ARE NOT YET AVAILABLE: Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP).

facility representative was informed of the MyChildCarePlan.org website; a
consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
