On September 16, 2025, at 09:40 AM, Licensing Program Analysts (LPAs) J. Duran and A. Carter conducted an unannounced Annual Required Inspection and met with Licensee Teresita Ramirez. LPAs disclosed the purpose of the inspection and granted entry into the facility. Licensee was provided with a LIC-126 Facility Entrance Checklist. All adults associated to the home were found to have criminal record clearance. There are currently 13 children enrolled. At time of the inspection there were 4 children and one assistant present.

The hours of operation are Monday through Friday from 6:00 AM to 6:00 PM. This is a single-story home which consists of (2) bedrooms, (2) bathrooms, kitchen, living room, family room, den, detached garage and backyard for playing. Areas used by the children includes: family room, den (main care ) and hallway bathroom. The kitchen is used for passing to backyard where children have access. LPAs observed safety gates and pocket doors to prevent children accessing off limit areas. Per Licensee, areas off-limits include (2) bedrooms, (1) bathroom, and garage.

The Parent Board had the following documents posted:

· Earthquake Preparedness
LPAs observed and reviewed the LIC 9040- Facility Roster and LIC 610- Facility Disaster Plan. The Disaster Drill log was current with the last drill conducted on July 07, 2025. Life saving devices such as Carbon Monoxide Detectors, Smoke Detectors, and Fire Extinguishers were observed and tested at time of inspection. The 2A10 BC Fire extinguisher was fully charged with tag dated 10/02/25.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

All areas identified on the facility sketch as accessible to children were inspected to ensure that they are clean and orderly with ventilation and heating for the safety and comfort of the children.

Napping equipment does not block entrances or exits. A variety of age-appropriate toys and learning materials were available for children in care, free of loose and sharp parts. There is a screened wall heater and screened fireplace with a locked cover in the living room, making it inaccessible to children in care. The hallway bathroom is utilized by the children. LPAs did not observe any hazards in the bathroom. Detergents, and medications are stored in an off-limits area of the home, and cleaning compounds are stored in a cabinet under the kitchen sink, latched an inaccessible to children in care. The Licensee states that there are no poisons in the home and understands poisons must be locked. The isolation area for sick children waiting to be picked up is the couch in the living room, supervised away from the other children in care. Per licensee, food is provided to children in care. LPAs reminded licensee that any food brought from the children’s homes shall be labeled with child’s name and properly stored or refrigerated. Currently there are no children enrolled needing medication administered, or children with allergies.

At 10:20 AM, LPAs observed (1) infant playing inside a play yard. Per licensee, the children will have access to the backyard for outside play. LPAs observed age appropriate toys available for the children such as basketball court and a plastic playhouse with slide, 2 foot pedal cars and tricycles. Per licensee, children are supervised while playing outside. LPAs observed perimeter fencing through-out the property. Play area is free of loose and sharp articles or objects and all trees, shrubs, and plants are maintained. LPAs did not observe any pools and/or bodies of water on the premises. LPAs observed (1) dog behind a gate in the backyard away from children.

Infant Care: Currently licensee has 1 infant under 12 months and 1 unfant under 24 months enrolled. Licensee was advised Infants shall not be swaddled in care, car seats shall only be used for transportation purposes and shall not be used for sleeping. LPAs discussed the Safe sleep regulation, including LIC 9227 Infant Sleep Plan for infants under 12 months, 15-minute sleep check documentation for infants 0 -24 months.

Children’s records were reviewed for (LIC) 282- Affidavit Regarding Liability Insurance, Immunization's Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights, and documentation of 15-minute Infant Sleep Check (0-24 months). All required licensing forms are on file.
Staff records were reviewed: Licensee's Pediatric First Aid / CPR is valid until: 09/28/26, had proof of required immunizations and proof of TB clearance. Licensee's Mandated Reporter training was completed in 07/28/24, valid for 2 years.
Per Licensee, there is 1 dog in the home, there are no weapons and/or firearms in the home, there are no smokers in the home, and there are no bodies of water on/or around the premises.

During the inspection, children present was observed to be treated with dignity and respect, observed to be receiving safe, healthful, and comfortable accommodations, furnishings, and equipment, and free from corporal and/or unusual punishment.

The following were discussed:
· Rooms that are off-limits need to be made inaccessible during operating hours. NO smoking, NO infant walkers, NO Johnny jumpers, NO saucer chairs, NO incline sleepers and any other item that falls into that category are permitted in the facility.
· Effective January 1, 2010, licensees of family child care homes are required to ensure that at least one staff member with current training in Pediatric CPR / First Aid is on site at all times when children are present.
· The fire extinguisher type 2A10BC must be serviced annually or as often as necessary. Smoke and carbon monoxide detectors should be checked, and batteries replaced as needed.
· Reporting Requirements: Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your home. Any unusual incidents or injuries must be reported to the Department within 24 hours via telephone and within seven (7) days in writing.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the Licensee Teresita Ramirez, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.



A notice of site visit was given and must remain posted for 30 days.
