25 | On November 19, 2024 at 9:05a.m, Licensing Program Analyst (LPA) Keneisha Dunlap arrived at the above facility for the purpose of an unannounced Case Management Visit. LPA Dunlap announced the purpose of the visit and was granted entry into the facility by Licensee Sofia Valdovinas. There 2 children present at the time of visit.
A case management visit was conducted on 11/19/24, Licensing Program Analyst (LPA) Keneisha Dunlap for the purpose of verifying the status of individuals that are associated to this facility. During this visit it was determined that all individuals who are associated have the required clearance and those who are no longer residing and/or working have been properly disassociated. There was one individual who did not have fingerprint clearance and the Licensee contacted the individual via phone to allow LPA Dunlap to explain to them that they must be re-fingerprinted in order to be present at the facility during operation hours. The Licensee requested for Guardian information in order to associate individual once they are fingerprinted. LPA Dunlap stated that they would send information via email tomorrow 11/20/2024.
From 9:40am-9:44am LPA conversation was translated with the interpreter service until Cristina Garcia arrived.
A notice of site visit was given and must remain posted for 30 days.
Appeal rights explained and given to Licensee Sofia Valdovinas.
Exit interview conducted and report was reviewed with the Licensee Sofia Valdovinas.
