25 | On November 19, 2024 at 10:00a.m, Licensing Program Analyst (LPA) Keneisha Dunlap arrived at the above facility for the purpose of an unannounced Case Management Visit. LPA Dunlap announced the purpose of the visit and was granted entry into the facility by Licensee Sofia Valdovinas. There are 2 children present at the time of visit.
During inspection on 11/8/24, The Licensee did not have power for more than 3 hours, and improperly stored boxes on top of refrigerators. As of 11/19/2024, the Licensee power has been restored. The boxes are still on top of area, however the area is no longer accessible to children in care as there in a barricade keeping children from off limits area.
During inspection on 11/8/24, The Licensee had thorned rose bush, had an accessible rake, ridge edges on canopy, birdcage droppings, and unsecured ladder on roof. The rose bush has been maintenance, the rake and ladder are on the side of the house, and the material for the canopy has been repaired with caulking. The birdcages have been removed and stored outside in an area inaccessible to children.
During inspection on 11/8/24, The Licensee did not have not safety barricade on stairs. The Licensee has baby gates barricading stairs making it in inaccessible to children.
During inspection on 11/8/24, The Licensee had chemicals and a knife accessible to children. The area where the knife was located is no longer accessible to children in care as there is a barricade keeping children from off limits area, and knife has been removed. A chemicals have been stored away inaccessible to children.
During inspection on 11/8/24, The Licensee did not have a current record of disaster/fire drills. The Licensee has current log with last date of 11/12/24. Page 1 of 2 |
