On November 26, 2024 at 9:30a.m, Licensing Program Analyst (LPA) Keneisha Dunlap arrived at the above facility for the purpose of an unannounced Case Management Visit. LPA Dunlap announced the purpose of the visit and was granted entry into the facility by Licensee Sofia Valdovinas. There are 9 children present at the time of visit, and 9 children enrolled.

From 9:50am-10:13am LPA conversation was translated with the interpreter service until Cristina Garcia arrived.

During inspection on 11/8/24, The Licensee was cited for a fire extinguisher with expired purchase date and/or service tag. Fire extinguisher has been updated with service tag of 11/21/24.



During inspection on 11/8/24, The Licensee was cited for not having immunization's for personnel in staff records. As of 11/22/24 staff file has up to date immunization's.

During inspection on 11/8/24, The Licensee was cited for not having Health & Safety certificates. The current staff have certificates.

During inspection on 11/8/24, The Licensee was cited for not having Health & Safety certificates. The current staff have certificates.

During inspection on 11/8/24, The Licensee was cited for not having CPR & First Aid certificates. All staff have certificates.

A notice of site visit was given and must remain posted for 30 days.

Appeal rights explained and given to Licensee Sofia Valdovinas.
