Licensing Program Analyst (LPA) Maddox met with Lead Teacher, Esperanza Montez and Linda Brown, ECE Coordinator for the purpose of conducting an unannounced Annual/Random inspection. This program operates on the grounds of Linda Verde Elementary School, classroom is located in the rear (East side of campus) in classroom #P2(2 half day programs & P5(full day). LPA toured and inspected all areas according to the facility sketch. Present today were 17 children and 3 staff persons. The facility consist of one classroom which operates two separate daily programs from 8:00am - 11:00 am and 12:30 pm - 3:30 pm.

**LPA noted age appropriate furniture, equipment, toys and materials. Classrooms were observed to be clean, safe and free of any Health or Safety hazards. Telephone service was verified as well as adequate heating, lighting, and ventilation. Children's belongings are kept in cubbies along the wall as you enter classrooms and labeled with each child's name. Drinking water is available inside the classroom in the form of water cooler with disposable cups.
**The children's bathroom is located within the classroom. The bathroom has 2 toilets and 1 sink with a portable located within the classroom.

The staff restroom is located immediately outside of the classroom. LPA observed the bathrooms to be clean and sanitary, with soap, toilet paper and paper towels readily available. Toilets and sinks are functioning properly and age appropriate.

**Outdoor play equipment was inspected for health, safety, good repair and age appropriateness. Center utilizes wood chips under climbing structure for cushioning material and there is a large shaded tarp area. Large play equipment and structure is securely anchored. The area was observed to be free of debris. Outside Drinking water is available in the form of water pitchers and disposable cups. Waiver on file to share outdoor space with Transitional Kindergarten with alternate outside schedules.

**LPA observed working smoke detectors, fire extinguishers, and carbon monoxide detectors throughout classroom. There is a log of emergency fire/disaster drills; sign in and out sheets contain full legal signatures; License is posted along with other forms that require posting after licensure. Names recorded of Staff and children files reviewed during this inspection.

**Lunches are provided by the School Cafeteria, menus observed and posted.



Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm - Director states there are no children present whom require IMS, but there is a plan in place.

Exit interview conducted, copy of report left with Lead Teacher. There were no violations noted as a result of this unannounced inspection.
