25 | On 8/24/2022 at 12:15pm Licensing Program Analyst (LPA), Denise Miranda conducted an unannounced Annual Required Inspection and was met by Licensee, Ana Maria Lopez. Also present was Licensee’s nephew and no children were present this time. Per Licensee, she has 7 children enrolled that are attending her day care in the afternoon. The days and hours of operation is from Monday – Friday – 6AM-6PM.
Licensee will submit via mail an update LIC279 Application form, licensee provided an updated LIC279B no later than 8/31/2022.
Per Sketch, the licensee's will submit no later than 9/2/2022 a revised Sketch floor plan and yard, showing the back house and all off-limit area, windows, doors etc. The house is one-story single-family home with 3 bedrooms, 3 bathrooms, kitchen, dining room with living room, den, converted garage (playroom) and one back house: with two bedrooms, one bathroom, kitchen with dining room and living room, per Licensee the back house is a off limit no children has access to this area. Also, the laundry area is located at the playroom area. Licensee is aware that all poisons product shall be inaccessible to the children all times.
The following areas are designated as follows and Licensee confirmed the: living room, dining room, kitchen to access the den room, converted garage to playroom and bathroom#3 located in the den room and backyard is accessible to the children in care..
The off-limit: Bedroom #1,#2 and #3 and bathroom##1# 2, back house.
There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition on the premises. No poisons were observed during the inspection. Detergents, cleaning compounds, medication and other hazardous items are made inaccessible.
Licensee has two pets: one cat and one dog with current immunization.
