25 | Licensing Program Analyst (LPA) R. Derraco conducted an unannounced required one year inspection to the above facility on 02/15/23. LPA arrived at the facility at 2:10 PM and met with resident, S2, who guided analyst on a tour of the facility. LPA observed licensee arriving to the home at 2:30 PM with S4. Licensee continued tour of the facility with LPA. Per Licensee, there are no children that are currently enrolled. There were 2 children present upon arrival. Per licensee, the two children present are her grand children and they live in the home with S2 and S3.
This is a two story home which consists of 4 bedrooms, 2 bathrooms, kitchen, dining room, 2 living rooms, a detached garage, a laundry room, front yard and backyard (fenced). LPA observed an off limits living space above the garage. The living space consists of a living room, one bedroom and a bathroom. Per licensee, the living space is occupied by her mother in law. Per licensee, the living space is part of the home's address however there is a separate entrance to the living space. Off limit areas include all four bedrooms, one bathroom, kitchen area, laundry room, upstairs living room, backyard, detached garage and living space built on top of the detached garage.
The main care area is located in the downstairs living room. LPA observed age appropriate toys, a pack an play, two couches, and a reclining chair. The fireplace in the main care areas was observed to be covered and locked. Emergency Disaster Plan, Parent’s Rights Poster and the Facility License were observed to be posted in the main care area. The bathroom was observed to have an operable sink, toilet and faucet. Locks were observed on the cabinet beneath the bathroom sink making the contents inaccessible. Locks were observed on all the cabinets in the off limits kitchen area. Per licensee, cleaning supplies are kept locked beneath the kitchen sink. The licensee states that she provides food for children in care. The front yard play area was observed with perimeter fencing, grass, and outdoor play equipment. Per licensee, isolation area for children showing signs of illness will be located in main care area while child awaits pick up. LPA did not
