On May 10, 2022 LPA Ortega arrived to conduct an annual inspection at facility and was informed by Licensee she decided to close as inactive last week due to Covid-19 pandemic. Licensee stated she intends to remain inactive starting May 7, 2022 until May 07, 2023. Licensee submitted a declaration form with signature requesting inactive status. Licensee is aware during inactive status no children can be care for until an annual is conducted.

Copy of this report, appeal rights and notice of site visit was provided to licensee.
