32 | Concerning the allegation of Child #1 (C1) sustaining multiple injuries at the facility, statements obtained from children, Licensee, and Staff #2 revealed C1 was running with a toy in his hand while Licensee, S2, and other children were present with the child in the back yard. It was disclosed that while C1 was running, he fell and sustained scratches on his face from the toy he was holding. Licensee applied ice pack and notified C1s parent at pick-up time. LPA inspected the area where C1 fell and did not observe any hazards that could have led to C1s accident. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the above allegations are the result of neglect and lack of supervision violation and personal rights violation. Therefore the allegations are unsubstantiated.
A notice of site visit was given to licensee and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
