The fire clearance was approved on 12/07/2022. Per Los Angeles Unified School District (LAUSD) Facility Use Authorization STAR will utilize Rooms, 34, 37, 42, and Cafeteria West. Auditorium authorized for use as needed. Fire Extinguisher was observed with a service date of 12/27/2021. LPA observed Fire Extinguishers in need of service.

LPA Lowe toured classrooms and outdoor area located at the facility. The facility will be located on Carpenter Elementary School Campus. The facility will have a total of 5 rooms, Rooms 34 is located in building A-1600, Room 37 is located in building AA-2944, Room 42 is located in building AA-3072, Cafeteria West is located in the Cafeteria building, and the Auditorium is located in the auditorium building.

Rooms 34 and 37 will be shared rooms with the elementary school using during school hours. Room 42 will be used exclusively by the licensed after school program.

Children will have access to 8 sinks, 10 toilets, and 6 urinals.

Indoor Activity Space measurements: Under Title 22 Regulation 101538.3(c) Indoor Activity Space for School-Age Children and Health and Safety Code Section 1596.806(a)(1) a measurement of the facility is not required.

Regulation 101538.3(c) School-age childcare programs that meet the requirements of Health and Safety Code Section 1596.806 are exempt from square-footage requirements for indoor activity space:

Outdoor Play measurements: Under Title 22 Regulation 101538.2(c)(1) Outdoor Activity Space for School-Age Children and Health and Safety Code Section 1596.806(b)(1) a measurement of the outdoor activity space is not required.

Regulation 101538.2(c)(1): (c) School-age child care programs that meet the requirements of Health and Safety Code Section 1596.806 are exempt from square-footage requirements for outdoor activity space, (1) Health and Safety Code Sections 1596.806(b), (b)(1) and (d) pertain to the exemption from square-footage requirements for outdoor activity space for school-age child care programs that meet the requirements of Health and Safety Code Section 1596.806.

Indoor/Children’s Area:

All outdoor and indoor passageways, stairways, inclines, ramps, open porches and other areas of potential hazard were observed to be free of obstruction. The child care center was observed be clean, safe, sanitary and in good repair. Floors of all rooms have a surface that is safe and clean. A comfortable temperature for children was observed. Furniture and equipment were observed to be maintained in good condition, free of sharp, loose or pointed parts. Tables and chairs were observed to be meet the needs of the children. All play equipment and materials used by children were observed to be age-appropriate. Children use backpacks as portable storage space for their clothing, personal belongings. All materials and surfaces accessible to children, including toys, were observed to be free of toxic substances.

LPA observed room 34 as a dance room with no tables or chairs. Per Director, room 34 will be used for yoga, stretching, and rainy-day activities.

LPA Lowe advised shared rooms with elementary school, facility shall conduct a walk through of shared space before the start of the after school program to ensure the rooms are free of hazards before the start of each day.

LPA Lowe observed disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children to be stored where inaccessible to children. LPA observed child proof latches on cabinets and with first aid kits and cleaning supplies. LPA observed first aid kit in room 42 to have a current edition of a first-aid manual, sterile first-aid dressings, bandages or roller bandages, adhesive tape, scissors, tweezers, thermometer, and antiseptic solution.

Per Director, the isolation area is located in the Room 42 behind the teachers desk. LPA observed isolation to be equipped with a mat available for an ill child. LPA advise the isolation area shall not be located in the kitchen area or the general-use toilet area.

Facility was observed with carbon monoxide detectors in good working order located in Room 42. LPA was able to hear a successful test. Facility uses cell phone as their working telephone service on the premises. Facility also uses walkie talkies for staff to communicate.

Per Director, Due to children arrive from their elementary classroom the children with obvious symptoms of illness including, but not limited to, fever or vomiting, will be sent home.

LPA advised smoking is prohibited on the premises of a child care center as specified in Health and Safety Code Section 1596.795(b).

LPA Lowe advised and reviewed with Director, CC Regulation 101516.5 Teacher – Child Ratio (a) In addition to Sections 101216.3(c), (e), (g) and (h), and notwithstanding Sections 101216.3(a), (b), (d) and (f), the following shall apply: (b) There shall be a staffing ratio of one teacher and one aide present to every 28 children in attendance. (1) A teacher shall supervise no more than 14 children or with an aide a maximum of 28 children.

LPA observed restroom for use by the STAR program to be located in building “Classroom Building #2” near the designated outdoor area. LPA observed all toilets and handwashing fixtures to be maintained in safe and sanitary operating condition. LPA advised additional equipment, aids and/or conveniences shall be provided as needed in centers that serve children with physical disabilities.

LPA Lowe advised due to the design of the bathroom visual supervision is limited when standing at the door. LPA advised a staff member will need to stand at the door way to supervise children waiting to use the restroom and a second staff member will need to stand inside the bathroom to supervise the bathroom. LPA took pictures of the bathroom layout.

Toilets used by school-age children shall provide individual privacy. Toilet facilities shall not be used simultaneously by children of both sexes.

LPA advised tot water temperature controls shall be maintained to automatically regulate temperature of hot water delivered to plumbing fixtures used by children to attain a hot water temperature of not less than 105

Notwithstanding (e) and (e)(1) above, handwashing fixtures shall not be required to deliver hot water.

Per Director, staff bathroom will be used for a child who is in the isolation area. This toilet and handwashing fixture shall be conveniently located in relation to the isolation area.

Outdoor:

LPA Lowe observed outdoor areas as Basketball court, Volleyball court, and Handball court all located outside of room 42. LPA observed a climbing structure located on campus near Rooms 10, 11, and 12, per Director the climbing structure will be off-limits to children participating in the STAR program.

LPA Lowe observed shaded rest area for the children. LPA Lowe observed the outdoor area to be free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard.

The playground was observed to be enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence was observed to be at least four feet high.

LPA advised any construction or equipment that could pose a hazard in the outdoor activity area shall be made inaccessible to children in care. This shall include, but not be limited to, incinerators, air- conditioning equipment, water heaters, and fuse boxes.

No pools or other bodies of water were observed on campus.

Health Related Services:

Per Director, if a child needs medication the parent/ guardian will complete a medication authorization form. Per Director, all medication will be in an original box with labeling. Medication will be stored in room 42, in a locked box located in the teacher locked desk drawer. LPA advised a refrigerator shall be used to store any medication that requires refrigeration.

LPA advised, Per CC Regulation 101226(e)(5) The licensee shall develop and implement a written plan to record the administration of prescription and nonprescription medication and to inform he child’s authorized

Food Service:

LPA advised all food shall be safe and of the quality and in the quantity necessary to meet the needs of the children. Each meal shall include, at a minimum, the amount of food components as specified by Title 7, Code of Federal Regulations, Part 226.20, (Revised January 1, 1990) Requirements for Meals, for the age group served. All food shall be selected, stored, prepared and served in a safe and healthful manner.

LPA Lowe observed standard size refrigerator in room 42. LPA Lowe observed thermometers in refrigerator and freezer. LPA advised all foods or beverages capable of supporting the rapid and progressive growth of microorganisms that can cause food infections or food intoxications shall be stored in covered containers at 45 degrees F (7.2 degrees C) or less.

LPA advised pesticides and other similar toxic substances shall not be stored in food storerooms, kitchen areas, food-preparation areas, or areas where kitchen equipment or utensils are stored.

LPA advised soaps, detergents, cleaning compounds or similar substances shall be stored in areas separate from food supplies.

Facility will provide afternoon snack to children. Food storge cabinet was observed in room 42.

Postings:

LPA Lowe observed the following postings in room 42: LIC 613A (9/96), the PUB 393 (8/02), Child Care Center Notification of Parents' Rights Poster, LIC 610 Emergency Disaster Plan with the telephone number of the local health department, information on child passenger restraint systems, and a contingency plan for action during fires, floods and earthquakes.

LPA observed STAR programs disaster and mass casualty plan of action.

LPA advised the licensee shall reveal each child care center license number in all advertisements in accordance with Health and Safety Code Section 1596.861.

Per Director, facility does not provide transportation. Children who will attend Carpenter STAR also attend Carpenter Elementary School.

The following items were discussed:

Facility representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA reviewed with facility representative the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

An exit interview was conducted with the above items discussed and a copy of appeal rights and this report was provided to the Director, Karina Rodriguez. Final license determination will be made upon review by the Licensing Program Manager and correction of the above issues.
