25 | On 3/8/2023 at 1:42pm, Licensing Program Analyst (LPA) Suzette Ornelas and Licensing Program Analyst (LPA) Maria Rendon arrived to the Zakaryan Family Child Care Home for a capacity increase inspection for a large family child care. LPA's met with licensee Gayane Zakaryan and assistant Rubik Yeghiazaryan who will serve 14 children ages 0-13 years old, Sunday through Saturday 6am-5:59am. Fire Clearance STD 850 was GRANTED on 2/3/2023. LPA's completed a tour of the home inside and outside.
Upon conducting the facility walk through, LPA's observed an infant asleep in bedroom 1 in a Pack n Play with loose article (blanket) over his body with his face fully visible. LPA's reminded licensee of the infant safe sleep regulations and ensuring there is nothing inside of the crib when infants are sleeping. Licensee immediately removed blanket from over child's body and stated blanket was over childs body per mothers request. LPA's reminded licensee that as a provider, she is to follow regulations and will be provided with information to share with parents regarding infant safe sleep today.
Per facility sketch and tour of the home, the following was observed and discussed with the applicant during the inspection:
Home is a 1 story dwelling. The family child care home consists of 3 bedrooms, one living room, one dining room, a den, 2 bathrooms and a detached car garage. There is an additional room behind the garage as well as a restroom. The driveway is located on the right side of the front door and leads to the garage and backyard. Parents will enter and exit through a pathway on the right side of the home leading to the backyard. In the backyard there is a stair casing that leads to a sliding door where children will enter and exit into the back of the home, the stair casing has a mesh gate at the bottom and a bar gate at the top.
Master bedroom, bedroom 1 and 2, bathroom located in the hallway, dining room/living room area, den, activity room (city permit approval provided) behind garage are on limits and accessible to children in care. Kitchen is off limits to children in care by the use of closed doors, safety gates and supervision. The facility is spacious enough to fit 14 children. |
