On 08/17/2023 Licensing Program Analysts (LPAs) Laticia Thompson and Jillinda Chandler met with Applicant Roshawn Hooper. The purpose of the visit was an announced pre-licensing inspection at Fairview Christian Academy & Enrichment Center. The applicant is requesting a license for 12 infants ages, infants ages 6 weeks - 17 months and 12 toddlers ages 18 months - 36 months. The child care center will operate Monday – Sunday 5:00 a.m. – 8:00 p.m. There is an approved fire clearance conducted by Walter Bergman of the L.A. City fire department. Applicant has a pending Preschool (197500098) and School-Age (195700099) application on file. The facility is on the Angeles Mesa Presbyterian Church premises, the church is a two story building, day care operation shall be conducted on the lower level only.

LPA observed the following of the indoor activity space:


Age appropriate furniture and equipment was observed in good repair
A variety of age-appropriate equipment, toys that provide and encourage Auditory stimulation, Visual stimulation, Tactile stimulation, and Manipulative skills.
LPA observed 5 cribs and cots for napping and were in good condition.
The room was equipped with central heating, air conditioning and windows for ventilation.
Fire extinguisher were last serviced on 4/21/2023.
Carbon monoxide detector were present during todays inspection
LPAs observed one changing table within arms reach of a sink.
The applicant plans to use potty chairs for potty training children, There shall be 1 potty chair per 5 training children.
Waste bins had tight fitting lids

LPA did not observed a refrigerator for formulas and foods capable of contamination to be stored.

First aid containing bandages, tweezers, thermometer and scissors were observed.
A parent board with the required posting was located in a common area for viewing.
Individual cubbies for children belongings were observed.

Indoor total measurements: 1085.65 (sq) feet (ft) divided by 35 sq ft = 31 children.

Food services: The applicant will provide meals and snacks. Per the applicant the main kitchen will not be used for food prepping. LPAs observed vermin feces in the cabinets. Applicant shall provide extermination reports prior to licensure. LPAs observed a food/activity table for feeding.



Outside activity space:
The Infant play yard was enclosed by a four feet high gate. In the front of the school. Applicant shall devise a plan to make bricked areas. water drain, sprinkler control, stair case and shrubbery inaccessible to children in care. Applicant shall ensure the wooden benches are free from splintering.
Toys and equipment were age appropriate in fair condition
Grass and mats were observed for cushioning
Canopies and trees provided shading, benches were observed for resting and water will be provided

Outdoor activity space measures:822.16 divided by 75 SQ FT = 10 children

Per todays inspection applicant shall be recommended for infant license with a capacity of 12 infants and 12 toddlers determined at the request of the applicant pending corrections and outdoor activities waiver approval.
  • Applicant shall provide extermination reports prior to licensure, and a declaration statement regarding the kitchen
  • LPA did not observed a refrigerator for formulas and foods capable of contamination to be stored
  • Applicant shall devise a plan to make bricked areas. water drain, sprinkler control, stair case and shrubbery inaccessible to children in care. Applicant shall ensure the wooden benches are free from splintering.
  • Applicant is hereby informed of all regulation pertaing to infant care can be found in Title 22, Division 12 Chapter 1 Sub chapter 02. Infant Care Centers

Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. LPA reviewed with applicant the LIC 311A, Records to Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed [applicant, licensee, or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment
