On 3/5/2025, Licensing Program Analysts (LPAs), Suzette Ornelas and Tatiana Bickham conducted an unannounced annual inspection at the address mentioned above. LPA met with Center Director, ROSHAWN T HOOPER and discussed the purpose of the visit. Facility also has a preschool (195700098) and school age(195700099) license on site. The Director guided LPA on a tour of the facility inside and out. Days and hours of operation are Monday through Friday 7:00am to 5:00pm.

LPA observed all required postings in a prominent area. LPA observed 37 children and 8 adults. All children are under supervision, including visual supervision, of a teacher at all times. Capacity and limitations as specified on the license are not being maintained. A walk through of the classroom area was conducted. LPA observed 3 licensed classrooms on the premises, 2 which are toddler classrooms.

All areas identified on the Facility Sketch were inspected.


Infant Classroom: 10 infants with a ratio of 3 adults
Toddler A: 15 children with a ratio of 2 adults (LPA Tatiana observed a third adult come walk into the classroom at 9:18am)
Per Director, 4 of the 15 children were preschool children.
Toddler B: 12 children with a ratio of 2 adults
Per Director, children that were observed in the Toddler B classroom by LPA's were children from the Preschool Program that were utilizing the Toddler B classroom for activity purposes.

A walk through of the classroom room space was conducted, the space was found to be clean and free from any potential hazards. Furniture was found to be in good repair and age appropriate. There is adequate heating, lighting and ventilation. Isolation area for sick students is in the Director’s office. LPA observed adequate arrangements for isolation and care of ill children.

The bathroom and toileting areas were inspected, LPA observed 4 toilets, 1 urinal and 3 sinks to accommodate the facility’s capacity. Toilets flush properly, toilet and sinks are reachable by the children in care. The restrooms have adequate toilet paper and paper towels available. The bathrooms were found to be clean. There is adequate lighting/ventilation in the bathroom area.

Playground equipment is in safe condition and free of hazards. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. Areas around high climbing equipment, swings and slides have cushioning material to absorb falls. All kitchen, food preparation and storage areas are clean and safe. Per Director, food program provides daily meals (breakfast, lunch, snack) for children enrolled. LPAs observed trays of food in the kitchen from food program (180). Per Director, sinks in kitchen are not utilized to prepare food. Solid waste storage containers have tight-fitting covers and are in good repair. Drinking water is readily available both indoors and outdoors via water dispensers located in the staff kitchen which is used to fill pitchers that are kept in the classrooms. Per Director and receipts obtained, facility utilizes water delivery service ReadyRefresh.com. The facility is equipped with a working smoke detector and carbon monoxide detector. LPA observed fire extinguishers (2A 10BC) located throughout the facility with service date of 3/20/2024. There is no swimming pool or other bodies of water on the premises. First Aid Kit was observed to be complete. Fire Drill log was observed to be complete and current (2/12/2025). There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. No poisons were observed during the inspection.

The facility has sufficient cots or mats for napping. Each mat or cot is occupied by only one child at time. LPA reviewed a sample of children’s files and observed files were complete. Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. Only one adult has a current CPR and Pediatric First Aid certificates all others were observed to be expired. The name of the child care center director or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department. LPA reviewed a sample of staff files and observed files were incomplete. Facility did not have staff transcripts readily available to view that could confirm teacher qualifications. Staff did not have current Mandated reporter Training (AB1207) Certificate; Tuberculosis, MMR and Tdap immunization's.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPA and Administrator discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.

Administrator was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA referred licensee to the Department website for lead: Lead Toxicity Prevention and Water Testing Information.

Administrator was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

LPAs did not observe LIC9224 Acknowledgement of Receipt of Licensing Reports due to Type A deficiency cited on 11/21/2024 in the children's files.

Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, 2 Type A and 6 Type B deficiencies are cited. (See LIC809-D)

Director was advised upon receive of this report, the report must be posted along with the notice of site visit for 30 days for parents to view. Licensee must inform the parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months via form LIC 9224 Acknowledgement of Receipt of Licensing Reports. If the Licensee fails to comply $100.00 civil penalty shall be assessed for each family enrolled and in attendance. Exit interview conducted with the licensee and a copy of this report was provided along with the appeal rights.

LPA's consulted with Director and provided Technical Support Program (TSP) Information and Flyer. Director agrees to work with TSP to get facility in compliance. LPA will submit referral request to TSP as soon as possible.

This report shall be made available to the public upon request. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director, ROSHAWN T HOOPER.
