On 11/21/2024, Licensing Program Analysts (LPA) Suzette Ornelas and LPA Laticia Thompson arrived at the above mentioned facility for the purpose of following up on a Case Management -Other visit conducted on 10/15/2024. LPAs were greeted by DIrector, ROSHAWN T HOOPER and guided on a tour of the facility.

During the visit conducted on 10/15/2024, Licensing Program Analyst (LPA) Laticia Thompson and Licensing Program Manager (LPM) Deborah Lowe observed that the Toddler room located next to the office had been converted into a teachers resources room. The Director stated classroom will be converted back to a Toddler classroom immediately.

LPAs arrived today to confirm that the room had been converted back to a Toddler Classroom. Upon entering the Toddler Classroom which had been converted into a teachers resource room, LPAs observed the classroom was converted back to a toddler classroom. LPAs observed 1 background cleared teacher (not associated to the infant license) supervising 12 toddlers in the Toddler Classroom B. LPAs observed 12 children sitting on a carpet. LPAs did not observe any tables or chairs present for the children to sit or eat at.

In the Infant Classroom, LPA Ornelas observed 8 Infants being supervised by 2 background cleared staff (1 which was not associated to the infant license).
While counting the children, LPA Ornelas observed 1 sleeping infant covered with a blanket. LPA informed Adult 2 (A2) of the Safe Sleep Regulation and A1 removed the blanket that was covering the child. LPA observed the Safe Sleep for your Baby Flyer posted on the wall near where infants sleep.

In the second Toddler Classroom A (next to the infant classroom), LPA Ornelas observed 15 toddlers being supervised by 3 background cleared adults (1 which was not associated to the infant license).

In total LPA observed 8 infants and 27 toddlers. Facility is operating over capacity. License specifies that the total capacity of the license serves 30 children which include 12 infants and 18 toddlers.

LPA informed Director, ROSHAWN T HOOPER that the facility was over capacity and obtained copies of children's roster for both the preschool and Infant license. Upon going back to the Toddler B classroom, LPA observed the 11 toddlers being relocated to the Preschool 3's classroom. LPA asked Director why children who were first observed in the toddler classroom B were being relocated to the Preschool 3's classroom, Director stated that the "daily schedule consists of utilizing both rooms if needed".

LPA obtained the names from Adult 1 (A1), of the 11 children that were moved into the Preschool 3's classroom as well as copies of the children's rosters. Upon review, LPA observed that some children were listed under the Infant/toddler roster (195700097) and others were listed under the preschool roster(195700098) and children are co-mingling.

Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, 1 type A deficiency and 3 Type B deficiencies are being cited. (See LIC809-D)

Licensee was advised upon receive of this report, the report must be posted along with the notice of site visit for 30 days for parents to view. Licensee must inform the parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months via form LIC 9224 Acknowledgement of Receipt of Licensing Reports. If the Licensee fails to comply $100.00 civil penalty shall be assessed for each family enrolled and in attendance. Exit interview conducted with the licensee and a copy of this report was provided along with the appeal rights.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Director, ROSHAWN T HOOPER including, but not limited to Provider Rights, Appeal Procedures.
