25 | On 3/28/24, at 1:15PM, Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an unannounced Case Management Visit to this facility. Upon arrival, LPA met with Center Director / Debbie A Leaf, Assistant Director / Christy Baumgartner and Infant/Toddler Instructional Coach / Shannon Burrington who guided LPA on a tour of the facility. There were 23 children with 10 staff observed present. LPA explained the purpose of today's visit is to follow-up on an incident that was reported to the Community Care Licensing (CCL) on 3/22/24.
On 3/21/24, at approximately 3:42PM, during transition from the Toddler classroom 1 to the playground, Staff 1 did not conduct a proper name-to-face roll call, nor a visual sweep of the classroom, resulting in Child 1 being left inside Toddler classroom 1, unsupervised for approximately 3 minutes. After returning back to the classroom at 3:45PM, with a group of children, Staff 2 discovered Child 1 inside the classroom, sitting in the book area looking at a book.
