On 02/29/2024 Licensing Program Analysts (LPA) Laticia Thompson met with Priscilla Rivera, Director and Mitul Sharma, Licensee. The purpose of the visit is a Prelicensing Inspection to ensure that health, safety and personal rights as required by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. There were 59 children and 9 staff members present during this inspection. This is an application to transition the facility to a single license caring for 20 Infants ages 6 weeks- 2 years and to include the Preschool license number 197494827 to accommodate 109 preschool children ages 2 years to entry into the 1st grade. The childcare center will operate Monday – Friday 6:30 AM. – 6:00 PM. The fire clearance was approved on 1/26/2024. LPA toured and inspected the on-limits areas. The facility has a total of 8 classrooms. Classroom # 6 will be dedicated to the Infant Program.
Indoor/Children’s Area:

The following was observed.



Parents will enter and exit through the front entrance of the facility. The entrance gate requires a code. Individuals may also gain access by pressing a button for assistance.

Parents currently sign in and out electronically under the Pro Care application. For best practice purposes, LPA advised facility there should be a plan in place on how the records would be backed up, and how they would be made available in the event of a power failure or disaster.

Preschool children will have access to 10 sinks and 9 toilets. Infants program does not have access to a sink or toilet.

Preschooler restrooms are located between classroom#1 and #2, classrooms #5 and #6, in classroom #7 and classroom#8. There are 3 staff restrooms located in the front office and 1 staff restroom located in the staff lounge area next to classroom #8. The toilets and sinks were observed to be maintained in safe and sanitary operating condition. Sink #2 located next to classroom #1 has a leak. All water fixtures used by children do not deliver hot water.

All the classrooms have water pitchers that are used to fill children personal water bottles. There are 3 functioning water fountains for outdoor activities.



There are operable carbon monoxide detector and First aid kits with the required materials in all classrooms. LPA observed an operable thermometer. There are several fire extinguishers with an inspection date of 02/22/24 with exception of classroom #5.

Furniture and equipment were observed to be maintained in good condition free of sharp, loose or pointed parts. Tables and chairs were observed to meet the needs of the children. Preschooler will nap on cots stored in various classrooms dedicated to the preschool. There are 4 cribs and 1 changing table located in the infant area. The facility is in the process of purchasing a refrigerator to store infants bottles and food.

There is a changing table/sink combination located in Room #8. LPA observed 1 changing table in classroom #7 that is not within arm’s reach of a sink.

There are cubbies in all classrooms for storage of children clothing and personal belongings.

All classrooms are equipped with air-conditioning and heating units to maintian proper ventilation . The classrooms were clean in good repair. Classrooms have adequate lighting. Windows were in good repair free of chipping paint, dirt, insects, or debris.

LPA observed trash cans with tight fitting lids. All play equipment and materials used by children were observed to be age-appropriate.

The facility is equipped with landline telephones service and survillance cameras located in all the classrooms.



Ill children will use the office and the staff restroom for isolation. The isolation area will be equipped with a cot or crib for ill children and shall be supervised by staff.

The licensee has developed and implemented a written plan to record the administration of prescription and nonprescription medications, to inform the child's authorized representative daily when such medications have been given. Medication is stored in the front office in containers inaccessible to children. Medication that requires refrigeration will be stored in the kitchen refrigerator inaccessible to children.

The kitchen was observed to be inaccessible to children. All foods and beverages that require storage were observed to be at a temperature of 45 degrees stored in the refrigerator located in the kitchen. The kitchen and food-preparation storage areas were observed to be clean and free of litter and rubbish. The refrigerator located in the kitchen has a thermostat to maintain temperature regulation. Soaps, detergents, cleaning compounds or similar substances are stored in the kitchen area separate from food supplies, located under the kitchen sink with safety latches. Disinfectants, cleaning solutions, and other items that are dangerous to children are stored in a cabinet located in kitchen with 2 safety latches.

The program provides breakfast, snack and lunch. Menus are in writing and are posted in advance in the classrooms and in an area accessible for review by the child's authorized representative. Copies of the menus as served shall be dated and kept on file for at least 30 days. Menus shall be made available for review by the child's authorized representative and the Department upon request.

The required postings were posted in the entry area outside of the office for parents and visitors to review. The facility has a disaster and mass casualty plan of action.

LPA did not observe any bodies of water on the premises. The facility does not provide transportation. There no firearms on the premises.

LPA reviewed and provided applicant will copies and posters of infant safe sleep, infant needs and service plans, infant sleeping equipment regulations.

Outdoor

The Infant outdoor activity space is physically separate from space used by children in the child care center.

The entire outdoor activity grounds are enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence is at least four feet high. The surface of the outdoor activity space has cushioning and artificial grass to absorb falls and prevent injury. The outdoor activity space was observed to be maintained in good condition There is adequate shade in the outdoor activity areas. LPA observed benches for resting.

The following items were discussed:



The director and licensee was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA reviewed with director and licensee the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

The licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

LPA discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed licensee, or facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

The following corrections are needed before a licensure will be considered.

· Place cushioning on spigot on play walk way area
· Repair leaking sink in restroom connected to classroom#1 and 2.
· Place a padded lock on gate where gardening and hazardous materials are stored.
· Provide proof of sink installed in infant room.

Corrections are due by March 26, 2024.

An exit interview was conducted with plan of corrected items discussed. LPA provided Mitul Sharma with appeal rights and a copy of this report. Final license determination will be made upon review by the Licensing Program Manager and corrections.
